PDCA循环护理管理在癌症患者PICC置管中的应用

Application of PDCA circular nursing management in PICC catheterization in cancer patients

目的在癌症患者PICC置管护理中实施PDCA循环管理,为癌症患者PICC置管的护理管理提供参考。方法选取该院肿瘤内科2015年1月~2016年12月收治的癌症患者126例作为研究对象,随机分为对照组和研究组各63例。对照组患者采用该院肿瘤内科PICC置管的常规护理管理方法,研究组患者采用PDCA循环护理管理模式,比较两组患者PICC置管并发症发生率及患者对PICC置管的满意度。结果实施PDCA循环护理管理的研究组患者的静脉炎、皮肤过敏、导管堵塞或脱出等并发症发生率低于对照组(P<0.05),但血栓的发生率没有明显差异(P>0.05),可能是由于本研究样本量较少造成的。对照组患者对PICC置管的满意度为62.8%,研究组患者为81.3%,实施PDCA循环护理管理的研究组患者对PICC置管的满意度明显高于对照组(P<0.05)。结论在癌症患者PICC置管中实施PDCA循环护理管理,能显著减少各种并发症的发生率,提高患者的满意度,值得临床推广应用。

Objective PDCA circulation management is implemented in PICC catheterization nursing for cancer patients,providing reference for the nursing management of PICC catheterization for cancer patients.Methods A total of 126 cancer patients admitted and treated in the department of oncology of the hospital from January 2015 to December 2016 were selected as the research objects,and randomly divided into the control group and the research group with 63 cases each.Patients in the control group were treated with the conventional nursing management method of PICC catheterization in the department of oncology of the hospital,and patients in the study group were treated with PDCA circulation nursing management mode. The incidence of complications of PICC catheterization and the satisfaction of patients with PICC catheterization were compared between the two groups.Results The incidence of phlebitis,skin allergy,catheter obstruction or prolapse was lower in the PDCA group than in the control group (P<0.05). However,there was no significant difference in the incidence of thrombosis(P>0.05). It may be due to the small sample size of this study. Patients in the control group were 62.8% satisfied with PICC catheterization,while those in the study group were 81.3% satisfied with PICC catheterization. Patients in the study group implementing PDCA circular nursing management were significantly more satisfied with PICC catheterization than those in the control group(P<0.05).Conclusions PDCA circular nursing management in PICC catheterization of cancer patients can significantly reduce the incidence of various complications and improve the satisfaction of patients,which is worthy of clinical application.
