高场磁共振IDEAL-IQ技术在急性胰腺炎中的应用价值研究 被引量:8

摘要 目的探讨高场磁共振IDEAL-IQ技术在急性胰腺炎中的应用价值。方法依据美国急性胰腺炎临床指南(诊断部分)搜集本院2014年12月至2016年12月的40例急性胰腺炎患者为实验组,临床需行医学影像学或其他相关检查无胰腺病变者40例为对照组。两组均采用IDEAL-IQ技术,在脂肪比图像和R2*弛豫率图像对胰腺头、体、尾部兴趣区多点多次测量脂肪含量和铁含量,并与CT检查结果进行对比,比较两组的脂肪含量和铁含量。比较高场磁共振IDEAL-IQ与CT检查的准确性。结果实验组的脂肪含量为(15.5±4.6)%,对照组的脂肪含量为(3.78±1.11)%,两者之间差异有统计学意义(P<0.05);实验组的铁含量为(598±101)Hz,对照组铁含量为(38±16)Hz,两者之间差异亦有统计学意义(P<0.05);高场磁共振IDEAL-IQ检查的准确率明显高于CT,具有统计学意义(P<0.05)。结论高场磁共振IDEAL-IQ序列一次扫描即获得多组图像,其中脂肪比像、R2*弛豫率像可以对急性胰腺炎患者的肝脏内的脂肪含量、铁含量行定量分析,对急性胰腺炎的诊断具有重要临床价值。 Objective To investigate the application value of high field magnetic resonance IDEAL-IQ in acute pancreatitis.Methods According to the American Acute Pancreatitis Clinical Guide(diagnostic part),40 patients with acute pancreatitis from December 2014 to December 2016 were enrolled in the experimental group.Medical imaging or other related examinations were performed without pancreatic lesions.40 cases were the control group.The IDEAL-IQ technique was used in both groups to measure the fat content and iron content in the head,body and tail regions of the pancreas multiple times in the fat ratio image and the R2*relaxation rate image,and compared with the CT examination results.The fat content and iron content of the group of subjects.Compare the accuracy of high field magnetic resonance IDEAL-IQ and CT examination.Results The fat content of the experimental group was(15.5±4.6) %,and the fat content of the control group was(3.78±1.11) %.The difference between the two groups was statistically significant(P < 0.05).The iron content of the experimental group was(598).± 101) Hz,the iron content of the control group was(38±16) Hz,the difference between the two was also statistically significant(P < 0.05);the accuracy of high field MRI IDEAL-IQ examination was significantly higher than that of CT.Statistical significance(P < 0.05).Conclusion High-field magnetic resonance IDEAL-IQ sequence can obtain multiple images in one scan.The fat ratio image and R2*relaxation rate image can quantitatively analyze the fat content and iron content in the liver of patients with acute pancreatitis.The diagnosis of pancreatitis has important clinical value.
