肾移植术后并发症的CT随访评价

The CT Evaluation of Complications After Kidney Transplantation

目的探讨同种异体肾移植术后并发症的CT诊断价值。方法回顾性分析本院2000年1月至2017年9月接受肾移植患者的临床及CT增强随访资料,分析患者肾移植术后并发症CT随访。结果本研究共纳入31例慢性肾功能不全尿毒症期接受肾移植患者,其中3例出现移植肾梗死,5例肾周积液,1例肾积水,4例肾周/输尿管积血,1例肾包膜下血肿,3例移植肾动脉管腔狭窄或闭塞,3例移植肾多发囊肿,3例移植肾多发点状结石,1例肾动脉与髂外动脉吻合口狭窄,2例肾皮质强化减弱,2例髂外动脉-移植肾动脉吻合口假性动脉瘤或血肿,1例移植肾萎缩,1例输尿管瘘,1例肾周脓肿,12例移植肾未见明显异常。结论 CT检查可以显示移植肾内外的多种结构,能够提供移植肾实质、血管、输尿管及周围组织的图像信息,具有重要的应用价值。

Objective To assess the complications after kidney transplantation using CT scaning.Methods We collected patients who received kidney transplantation from Jan 1,2000 to Sept 2017 in terms of their clinical information and CT images.Their pre-contrast and contrast-enhanced CT images were analyzed retrospectively.Results Overall,there were 31 patients who underwent kidney transplantation,who enrolled in this study.Of these,renal infarction was observed in 3 cases;perirenal fluid in 5 cases;hydronephrosis in 1 cases;perirenal or periureteral hemorrhage in 4 cases;renal subcapsullar hematoma in 1 case;stenosis or occlusion of renal artery in 3 cases;multiple renal cysts in 3 cases;kidney stone in 3 cases;anastomotic stenosis between the renal artery and external iliac artery in 1 case;decreased renal enhancement in 2 cases;anastomotic pseudoaneurysm or hematoncus between the renal artery and external iliac artery in 2 cases;nephrarctia in 1 case;graft urinary fistula in 1 case;perinephric abscess in 1 case;and normal kidney transplantation in 12 cases.Conclusion CT examination can show various structures inside and outside the transplanted kidney,and can pro vide image information of transplanted renal parenchyma,blood vessels,ureters and surrounding tissues,which have important application value.
