溃疡性结肠炎与克罗恩病患者生存质量对比研究

A Comparative Study On The Quality Of Life Of Patients With Ulcerative Colitis and Crohn's Disease Based On The Theory Of Tonifying Earth in Traditional Chinese medicine

目的:比较溃疡性结肠炎(UC)与克罗恩病(CD)患者的生存质量,并基于中医补土理论进行探讨。方法:应用一般资料调查表、情绪自评量表、一般健康状况问卷(SF-36)和炎症性肠病生存质量问卷(IBDQ),对54例UC患者和40例CD患者进行问卷调查,所得数据通过两独立样本t检验、卡方检验、两两进行比较。结果:在生存质量对比上,CD患者的IBDQ总分和各项评分均优于UC患者,其中IBDQ总分、肠道症状、全身症状、情感功能、生理职能、精力和心理健康等指标的比较差异有统计学意义。结论:CD患者生存质量明显优于UC患者,从中医补土角度认识,CD病机为太阴脾虚,病在土位,土具坤德故病程缠绵,临床症状变化不剧烈,对生存质量影响较小;而UC病机为厥阴血分受邪,太阴脾土不足,病在木位,风善行而数变,所以UC患者多病情反复,临床症状变化较快,而且以出血为主,对生存质量的影响较大。

Objective:To compare the quality of life (QOL)of patients with Ulcerative colitis(UC)and Crohn's disease(CD),the results were discussed based on earth-invigorating thoughts of Traditional Chinese Medicine.Methods: Questionnaire was used to collect 54CD patients and 40UC patients'data including general information questionnaire,self rating anxiety scale (SAS ),self rating depression scale (SDS ),MOS 36-item Short Form Health Survey (SF-36),inflammatory bowel disease questionnaire (IBDQ),comparisons between CD and UC were performed by means of Chisquare test and Two-tailed t-test.Results:As for QOL,CD patients scored significantly higher than their UC counterparts on most domains of IBDQ,wherein there were significantly differences on IBDQ total score,bowel symptoms,systemic symptoms,emotional function,role-physical,vitality and mental health.Conclusion:QOL of CD patients is better than those of UC,from the aspect of spleen-earth supplement in Chinese medicine,the pathological machanism of CD is taiyin syndrome with spleen deficiency,which generated in earth,earth is possessed of kun merit,thus the CD patients are commonly affected in a chronic way with slow progress and inapparent changes of clinical symptoms,leading to smaller impact to QOL of the patients.Contrarily,the pathological machanism of UC is Jueyin syndrome with blood affected by evil and Taiyin meridian with a spleen insufficiency,which generated in wood,the wind is changeable and dynamic,so the UC patients experience unstable health condition and recurrent clinical symptoms with bleeding,contributing to a worse performance on QOL.
