基于RBRVS绩效评估系统的本土化路径研究

Research on Localization Path of RBRVS Performance Evaluation System

目的:以RBRVS为理论依据,构建临床医师的绩效评价指标体系,并提出应从"指标-实测-分析-评价"4个角度探讨RBRVS本土化研究路径。方法:文献研究法、头脑风暴法、半结构化访谈法、问卷调查法、德尔菲法和层次分析法。结果:现况调查结果可以看出,当前收减支的绩效分配制度不合理、医生的压力大、劳动付出与回报不对等、技术和风险等要素无法体现;同时构建了包括时间消耗(0. 1600)、劳动付出(0. 2772)、技术投入(0. 4669)和风险压力(0. 0953)在内的4个一级指标和13个二级指标的临床医师绩效评价指标体系,并提出了RBRVS的本土化研究路径。结论:该指标体系可为确定本土化的医疗服务项目的相对价值和科学计算医师的绩效薪酬提供依据,该路径的提出也可为国内医院进行绩效薪酬改革提供思路。

Objective On the basis of resource based relative value scale as the theoretical basis,constructing performance evaluation system for the clinician,and put forward should be"index-measured-analysis-evaluated"the four angles to explore RBRVS localization research way. Methods Literature research,brainstorming,semi-structured interview,questionnaire survey,Delphi method and analytic hierarchy process. Results the survey results can be seen that the current spending cuts the performance of the distribution system is not reasonable,the doctor’s pressure,unequal,technology and labor effort and reward risk elements such as unable to reflect. Build at the same time including the time consumption( 0. 1600),the labor cost( 0. 2772),the technology( 0. 4669),risk and pressure( 0. 0953) and the four level indicators and 13 secondary indicators of performance evaluation system for the clinician,and put forward the RBRVS localization of research way. Conclusion The localization of the index system could be used to determine the relative value of medical services and provides the reference for the performance pay of scientific computing physicians,path can also be put forward for domestic hospital performance pay reform train of thought.
