血清降钙素原、C反应蛋白及外周血白细胞计数联合检测在儿童呼吸道感染性疾病诊断中的应用 被引量:22

摘要 目的探讨血清降钙素原(PCT)、C反应蛋白(CRP)及外周血白细胞(WBC)计数联合检查在儿童呼吸道感染性疾病诊断中的应用。方法选取本院儿科收治的呼吸道感染疾病患儿120例为研究对象,按感染病原体分为细菌感染组(62例)和非细菌感染组(58例)。细菌感染组中革兰氏阳性菌感染28例(革兰氏阳性菌组),革兰氏阴性菌感染34例(革兰氏阴性菌组),另选同期在本院体检的健康患儿50例为对照组,分别检测各组PCT、CRP及WBC水平。结果细菌感染组WBC计数及血清PCT、CRP水平显著高于对照组和非细菌感染组(均P<0.05);革兰氏阴性菌组PCT水平显著高于革兰氏阳性菌组(P<0.05),而WBC、CRP水平两组间比较差异无统计学意义(均P>0.05);细菌感染组治疗72 h后WBC、PCT、CRP水平较治疗前显著下降(均P<0.05)。结论血清PCT、CRP及外周血WBC计数联合检查可对患儿呼吸道病原微生物感染类型做出初步诊断,且血清PCT和CRP水平可作为抗感染治疗效果判断的重要参考依据。 Objective To explore clinical application of combined detection of serum procalcitonin(PCT),C-reactive protein(CRP) and peripheral blood white blood cell(WBC) count in diagnosis of children with respiratory tract infectious disease.Methods A total of 120 children with respiratory tract infectious disease were selected from department of pediatrics in our hospital,and were divided into bacterial infection group(62 cases)and non-bacterial infection group(58 cases)according to the different pathogen types of infection.in bacterial infection group,gram positive bacterial infection 28 cases(gram positive bacterial group),gram negative bacterial infection 34 cases(Gram negative bacterial group),meanwhile,50 healthy children who had gone through physical examination in our hospital during the same period were selected as control group,serum levels of PCT,CRP and WBC count were detected.Results The levels of PCT,CRP and WBC in bacterial infection group were significantly higher than those in non-bacterial infection group and control group(all P<0.05);the level of PCT in gram positive bacterial group was significantly higher than that in gram negative bacterial group(P<0.05),but the levels of WBC and CRP in two groups were not statistically significant(all P<0.05);the levels of WBC,PCT and CRP after 72 hours treatment in bacterial infection group were significantly decreased compared with before treatment(all P<0.05).Conclusion The detection of serum PCT,CRP and peripheral blood white blood cell count can preliminarily estimate the etiological types of respiratory tract infection,and the levels of serum PCT,CRP is the important references to evaluate the effect of anti-infection treatment.
