2017年无锡市滨湖区初中寄宿新生和高中新生结核菌素试验结果分析

Analysis of the Purified protein derivative(PPD) skin test results among grade one boarders in junior high school and senior high school in Binhu district of Wuxi in 2017

目的了解无锡市滨湖区初中寄宿新生和高中新生结核病感染情况,为学校结核病筛查提供科学依据。方法对2017年滨湖区2 172名初中寄宿新生和高中新生进行结核菌素试验(PPD),对强阳性者进行胸部X线摄片检查。结果初中寄宿新生和高中新生两组间的一般阳性+中度阳性率、强阳性率、总阳性率差异均有统计学意义(χ~2值分别为31.3、8.12、41.57,P值均<0.01)。其中一所有部分新疆学生的高中与其他高中新生的总阳性率、一般阳性+中度阳性率均有统计学意义(χ~2值分别为270.94、324.49,P值均<0.01)。高中新生男生总阳性率、强阳性率均低于女生,差异均有统计学意义(χ~2值分别为8.51、6.28,P值均<0.05)。对65名PPD强阳性者进行胸片检查,未发现活动性肺结核患者。结论无锡市滨湖区高中新生结核菌感染状况不容乐观,应将结核病筛查纳入新生体检,并加强对新疆学生的结核病监测管理,以早期发现肺结核病人,防止结核病在学校暴发流行。

Objective To investigate the situation of mycobacterium tuberculosis infection among grade one boarders in junior high school and senior high school in Binhu district of Wuxi in 2017 and then provide scientific basis for tuberculosis(TB) screening in school.Methods Purified protein derivative(PPD) skin tests were performed among 2172 students in grade one boarders in junior high school and senior high school in Binhu district of Wuxi.The students whose results of PPD test were strong positive would further be checked by chest X-ray radiography.Results The differences of weakly positve + moderately positive rate,strong positive rate and total positive rate between grade one boarders in junior high schools and senior high schools were significant.The differences of weakly positive +moderately positive rate and total positivity between senior high schools with students from Sinkiang and other senior middle school were significant.The total positivity and strong positivity of males’ are significantly lower than the females’ in senior middle school.65 students whose results of PPD test were strong positive were checked by chest Xray radiography and no active tuberculosis patient was checked out.Conclusion The situation of mycobacterium tuberculosis infection is not optimistic in senior high school in Binhu district of Wuxi.In order to detect tuberculosis patients earlier and prevent TB outbreak in school,tuberculosis(TB) screening should be included in the physical examination at admission and the monitoring of tuberculosis(TB) in Sinkiang students should be strengthened.
