SPECT/CT融合显像对诊断早期股骨头缺血性坏死的临床研究

Clinical research of using SPECT/CT fusion imaging in the early diagnosis of avascular necrosis of femoral head

目的评价SPECT/CT融合显像在早期股骨头缺血性坏死中的临床诊断价值。方法通过临床拟诊为"股骨头缺血性坏死"成年患者67例,共82个股骨头进行融合显像检查,并结合最终临床诊断分析同单纯SPECT平面显像、CT检查的诊断效能。结果 SPECT/CT融合显像诊断股骨头缺血性坏死的诊断灵敏性96.9%(63/65)、特异性88.2%(15/17)、准确性95.1%(78/82),与SPECT平面显像相比较诊断灵敏性、特异性及准确性差异均有统计学意义(P<0.05);与CT检查相比较灵敏性和准确性差异有统计学意义(P<0.05)。结论 SPECT/CT融合显像对早期股骨头缺血性坏死的诊断优于单纯SPECT和CT检查,具有重要的临床意义。

Objective To evaluate the diagnostic value of SPECT/CT fusion imaging in the early diagnosis of avascular necrosis of femoral head.Methods Sixty-seven patients who were doubted avascular necrosis of femoral head,a total of eighty-two emoral heads,were examined by SPECT/CT fusion imaging.All results were compared with final clinical findings.The diagnositic efficacy indicators of SPECT,CT and SPECT/CT fusion imaging were compared and analyzed.Results The sensitivity,specificity and accuracy of SPECT/CT fusion imaging were96.9%(63/65),88.2%(15/17) and 95.1%(78/82),respectively.The sensitivity,specificity and accuracy of SPECT/CT fusion imaging were higher than those of SPECT imaging,there was a statistically significant difference between these two methods(P<0.05).The sensitivity and accuracy of SPECT/CT fusion imaging were higher than those of CT imaging,there was a statistically significant difference between these two methods(P<0.05).Conclusion SPECT/CT fusion imaging have higher sensitivity and accuracy compared with individual SPECT imaging or CT imaging and fusion imaging have significant clinical value in the early diagnosis of avascular necrosis of femoral head.
