帕瑞昔布钠减少肺移植后的缺血-再灌注损伤

Parecoxib sodium attenuates the ischemia/reperfusion injury after lung transplantation

摘要 目的探讨帕瑞昔布钠(parecoxib sodium,PS)对于肺移植后缺血再灌注损伤的影响。方法24只受体大鼠随机分为:假手术组(shamp group,S),磷酸缓冲盐溶液组(phosphate-buffered saline group,PBS)和帕瑞昔布钠组(parecoxib sodium group,PS)。假手术组仅接受麻醉; PBS组肺移植再灌注后立即注射生理盐水; PS组再灌注后立即注射帕瑞昔布钠。再灌注24 h后测量氧合指数,移植肺湿干重比和蛋白浓度,血清中的TNF-α、IL-1β、IL-6和IL-10浓度,观察移植肺组织学损伤情况。结果与PBS组比较,PS增加再灌注后氧合指数,降低移植肺湿干重比和蛋白浓度; PS减少血清中TNF-α、IL-1β、IL-6的生成、促进抗炎因子IL-10的释放,减轻肺移植后组织学损伤。结论 PS可以减轻肺移植缺血再灌注损伤,其作用机制与改善肺移植后肺泡-毛细血管通透性,抑制炎症反应有关。 Objective To examine the effects of parecoxib sodium( PS) on lung ischemiareperfusion injury( LIRI). Methods Twenty-four recipients were randomized into 3 groups: S group,PBS group and PS group. Rats in sham group only received anesthesia. Rats in PBS and PS groups received left lung transplantation and received saline or PS immediately after reperfusion. The partial pressure of oxygen to fraction of inspiratory oxygen( PaO2/Fi O2) ratio,wet-to-dry weight ratio and protein levels in transplanted lung and inflammation-related factors levels in serum were examined. Histological change of transplanted lung were analyzed. Results Compared with PBS group,the PaO2/Fi O2 ratio was dramatic increased,and the wet-todry weight ratio and protein level were significantly decreased by PS after reperfusion. The lung histological injury were attenuated by PS. The pro-inflammatory factors in serum were downregulated,whereas IL-10 was up-regulated in PS group. Conclusion PS ameliorates LIRI after lung transplantation. The protection of PS is partly associated with anti-inflammation.
