我国城区门诊支气管哮喘患者疾病严重程度及合并症的调查分析 被引量:5

Disease severity and comorbidities of outpatients with asthma from urban area in China

摘要 目的调查我国城区门诊支气管哮喘患者疾病严重程度及合并症。方法 2015年10月至2016年5月于我国内地(除西藏外)30个省、自治区、直辖市的30家三级甲等医院进行面对面问卷调查研究,符合入选标准的门诊哮喘患者纳入研究。研究内容包括:(1)一般资料;(2)吸烟状态;(3)疾病严重程度;(4)合并症。结果本次调查共纳入3 875例门诊哮喘患者。根据GINA指南,疾病严重程度分级:间歇状态52.5%(2 033/3 875),轻度持续24.5%(951/3 875),中度持续16.9%(656/3 875),重度持续6.1%(235/3 875)。体重超重率32.9%(1 274/3 875),肥胖率10.3%(401/3 875);吸烟率为20.1%(777/3 875),其中现行吸烟率为7.7%(298/3 875)。肥胖、吸烟均与哮喘控制不佳相关。75.9%(2 941/3 875)的哮喘患者有合并症,依次为:过敏性鼻炎43.4%(1 682/3 875),高血压病16.4%(634/3 875),鼻窦炎8.7%(338/3 875),慢性阻塞性肺疾病7.3%(283/3 875),支气管扩张症3.0%(118/3 875),鼻息肉2.9%(114/3 875)。结论门诊就诊的哮喘患者轻度占多数。哮喘患者共患过敏性鼻炎、支气管扩张的比例显著高于人群平均水平。肥胖、吸烟均与哮喘控制不佳相关。 Objective To investigate the disease severity and comorbidifies in outpatient with asthma from urban area in China.Methods A face-to-face,questionnaire-based survey was carried out in outpatient department in 30 general hospitals from 30 provinces in China mainland except for Tibet during October 2015 to May 2016,and asthma patients who meet the including criteria were enroUed.Data of demographic characteristics,smoking status,disease severity,and comorbidities were collected.Results A total of 3875 cases were included.According to GINA criteria,the proportion of diseases severity was as following:intermittent status 52.5%(2033/3 875),mild persistent 24.5%(951/3 875),moderate persistent 16.9%(656/3 875),and severe persistent 6.1%(235/3 875).The overweight rate was 32.9% (1 274/3 875),the rate of obesity was 10.3%(401/3 875),and the smoking rate was 20.1%(777/3 875).Obesity and smoking were related to poor asthma control.75.9%(2 941/3 875)of the patients had comorbidities:allergic rhinitis 43.4% (1682/3 875),hypertension 16.4%(634/3 875),nasosinusitis 8.7%(338/3 875),chronic obstructive pulmonary disease 7.3% (283/3 875),bronchiectasis 3.0%(118/3 875),and rhinopolypus 2.9%(114/3 875).Conclusions The majority of asthma patients from outpatient department are mild asthma patients.The proportion of allergic rhinitis and bronchiectasis in asthma patients is higher than mean level.Asthma patients with comorbidities of obesity and smoking is related to poor asthma control.
