麻杏蒌石汤联合布地奈德雾化吸入治疗AECOPD的疗效及对Th1/Th2失衡的影响 被引量:11

EFFECT OF MAXINGLOUSHI DECOCTION COMBINED WITH BUDESONIDE INHALATION ON AECOPD AND ITS EFFECT ON TH1/TH2 IMBALANCE

摘要 目的探讨麻杏蒌石汤联合布地奈德雾化吸入治疗慢性阻塞性肺疾病(COPD)急性加重期(AECOPD)的疗效及对Th1/Th2失衡的影响。方法将92例AECOPD患者随机分为观察组和对照组,每组46例。对照组采用雾化吸入布地奈德治疗。观察组在对照组基础上,采用麻杏蒌石汤治疗。于治疗前后,检测二组患者的免疫功能指标(Th1细胞、Th2细胞、Th1细胞/Th2细胞)和肺功能指标(FEV1、FEV1/FVC、PEF、FEV1%预测值)的水平变化。结果观察组的总有效率明显高于对照组,差异有统计学意义(P<0.05);观察组治疗后的Th1细胞、Th1细胞/Th2细胞较治疗前均明显升高(P<0.05);治疗后,观察组的Th1细胞、Th1细胞/Th2细胞明显高于对照组,差异有统计学意义(P <0.05)。二组治疗后的FEV1、FEV1/FVC、PEF、FEV1%预测值较治疗前均明显升高(P<0.05);治疗后,观察组FEV1、FEV1/FVC、PEF、FEV1%预测值升高的程度显著优于对照组,差异有统计学意义(P<0.05)。结论麻杏蒌石汤联合布地奈德雾化吸入能显著改善AECOPD患者的肺功能,调节Th1细胞/Th2细胞平衡。 Objective To investigate the effect of Maxingloushi decoction combined with budesonide in the treatment of acute exacerbation(AECOPD)of chronic obstructive pulmonary disease(COPD)and the effect on the imbalance of Th1/Th2.Methods Ninety-two patients with AECOPD were divided into the observation group and the control group,46 cases in each group.The control group was treated with nebulization of budesonide.The observation group was treated with Maxingloushi decoction on the basis of the control group.Before and after treatment,the immune function indexes(Th1,Th2,Th1/Th2)and pulmonary function indexes(FEV1,FEV1/FVC,PEF,FEV1%predicted value)of the two groups were measured.Results The total effective rate of the observation group was significantly higher than that of the control group.The difference was statistically significant(P<0.05).The Th1 and Th1/Th2 in the observation group were significantly higher than those before treatment(P<0.05).After treatment,the Th1 and Th1/Th2 in the observation group were significantly higher than those in the control group.The difference was statistically significant(P<0.05).The FEV1,FEV1/FVC,PEF and FEV1%predicted value of the two groups were significantly higher than those before treatment(P<0.05).After treatment,the levels of FEV1,FEV1/FVC,PEF and FEV1%predicted value in the observation group were significantly higher than those in the control group.The difference was statistically significant(P<0.05).Conclusion Maxingloushi decoction combined with budesonide inhalation can significantly improve lung function and regulate Th1/Th2 balance in AECOPD patients.
