复方苦参注射液联合顺铂治疗恶性胸腔积液的临床研究

Clinical study of Compound Radix Sophorae Flavescentis Injection Combined with Cisplatin in Treatment of Malignant Pleural Effusion

目的:观察复方苦参注射液联合顺铂治疗恶性胸腔积液的临床疗效。方法:将2017年1月—2018年6月收治的肺癌合并恶性胸腔积液的患者50例,随机分为对照组和治疗组各25例。对照组引流后予顺铂加生理盐水腔内灌注治疗;治疗组在对照组的基础上加用复方苦参注射液腔内灌注治疗。治疗1个月后评价患者疗效。结果:治疗组总有效率(72.00%)明显优于对照组(40.00%),P<0.05。咳嗽、气促、纳差症状积分,治疗组治疗后明显优于对照组治疗后,P<0.05。治疗组不良反应发生率明显优于对照组,P<0.05。治疗组治疗后CD_3^+、CD_4^+、CD_4^+/CD_8^+、NK细胞较治疗前明显上升,CD_8^+下降(P<0.05)。治疗后两组比较,治疗组CD_3^+、CD_4^+、CD_4^+/CD_8^+及NK细胞较对照组升高,CD_8^+较对照组下降(P<0.05)。结论:复方苦参注射液联合顺铂治疗恶性胸腔积液临床疗效较好,可明显改善患者的生活质量和降低不良反应。

Objective:To objective the clinical efficacy of compound matrine injection combined with cisplatin in the treatment of malignant pleural effusion.Methods:Fifty patients with lung cancer complicated with malignant pleural effusion were randomly divided into control group and treatment group with 25 cases in each group.The control group was treated with cisplatin plus normal saline after drainage.The treatment group was added with compound matrine injection on the basis of the control group.The curative effect was evaluated 1 months after treatment.Results:The total effective rate of the treatment group(72.00%) was significantly better than that of the control group(40.00%),P<0.05.The scores of cough,shortness of breath and poor appetite in the treatment group were significantly better than those in the control group(P<0.05).The incidence of adverse reactions in the treatment group was significantly better than that in the control group,P<0.05.After treatment,CD3^+,CD4^+,CD4^+/CD8^+,NK cells in the treatment group were significantly higher than those before treatment,and CD8^+ was decreased(P<0.05).After treatment,CD3^+,CD4^+,CD4^+/CD8^+ and NK cells in the treatment group were higher than those in the control group,and CD8^+ was lower than those in the control group(P<0.05).Conclusion:Compound Sophora Flavescens Injection combined with cisplatin in the treatment of malignant pleural effusion has better clinical efficacy,can significantly improve the quality of life of patients and reduce adverse reactions.92257X 2019,(01):87-89
