速冻低温倒置离心法去除血浆乳糜微粒的效果及其对传染性指标检测的影响 被引量:6

摘要 目的探讨速冻低温倒置离心法去除血浆乳糜微粒(CM)的效果,以及CM对献血者血液检测结果的影响。方法收集本站制备的血浆中因乳糜血报废的50袋(200 m L/袋),从每袋中分出10 m L血浆共50份,作为不分离CM组(对照组);剩余的血浆50份,作为分离CM组;采用全自动生化分析仪和原装试剂检测2组血浆中的三酰甘油(TG)、总蛋白(TP)和丙氨酸氨基转移酶(ALT),全自动血凝仪和原装试剂检测因子Ⅷ(FⅧ)和纤维蛋白原(Fib),分别采用2种酶联免疫吸附试验(ELISA)试剂检测乙型肝炎病毒表面抗原(HBs Ag)、丙型肝炎病毒抗体(抗-HCV)、人类免疫缺陷病毒抗体(抗-HIV)和梅毒螺旋体(抗-TP)。结果分离CM组与对照组比较:TG(mmol/L)为3. 56±0. 70 vs 3. 13±0. 66(P<0. 01); TP、FⅧ、Fib和ALT相近(P>0. 05); HBs Ag试剂1检测的S/CO值为0. 19±0. 23vs 0. 10±0. 11(P<0. 01),试剂2的S/CO值相近(P>0. 05);抗-HCV试剂1和试剂2的S/CO值比较均无明显差异(P>0. 05);抗-HIV试剂1和试剂2的S/CO值比较均无明显差异(P> 0. 05);抗-TP试剂1检测的S/CO值相近(P>0. 05),试剂2的S/CO,值为0. 12±0. 18 vs 0. 07±0. 12(P<0. 05)。结论速冻低温倒置离心法可有效降低轻、中度乳糜血中的CM,保持分离血浆FⅧ活性;轻、中度乳糜血对ALT的结果影响小,但对ELISA方法检测其他4项献血者血液传染性指标的S/CO值均有所增高。 Objective To evaluate the performance of chylomicron removal from plasma by rapid-cooling inverted-centrifugation,and then discuss its influence on plasma quality for blood donors. Methods Fifty bags of plasma(200mL/bag,which were to be discarded due to chylemia) were collected from Longgang central blood station. Among them 10 mL plasma was separated to be the CM group(control group). And the others were assigned to the CM-removed group. Triglyceride(TG),Total protein(TP) and alanine tranaminase(ALT) were tested in both groups with automatic biochemical analyzer;Ⅷ factor and fibrinogen(Fbg) were tested by automatic hemaglutination instrument;hepatitis B virus surface antigen(HBs Ag),hepatitis C virus antibody(HCV-Ab),human immunodeficiency virus antibody(anti-HIV) and syphilis spirulina(antitp) were tested by two different Enzyme linked immunoadsorption test(ELISA) reagents. Results TG(mmol/L) were 3. 56±0. 70 vs 3. 13±0. 66(P<0. 01),TP、FⅧ、Fib and ALT had no significant differences(P>0. 05) between the two groups. HBs A S/CO were 0. 19±0. 23 vs 0. 10±0. 11(P<0. 01) with reagent 1,and there were no differences with reagent 2(P>0. 05).There are no obvious differences(P > 0. 05) between antiHCVreagents(P > 0. 05),and among anti-HIV regents(P >0. 05) The S/CO of anti-TP reagents 1 were near in both groups(P>0. 05),and the S/CO of anti-TP reagents 2 were 0. 12±0. 18 vs 0. 07±0. 12(P<0. 05). Conclusions The rapid-cooling inverted-centrifugation could efficiently decrease CM concentration in mild,moderate chylomicron blood,and maintain plasmaⅧ factor activity. Mild,moderate chylemia has had little impact on ALT,but other 4 infectious indexes increased in ELISA tests.
