皮肤浆细胞增生症1例

A Case of Cutaneous Plasmacytosis

患者男,53岁,面颈部、腋窝、躯干红褐色斑片3年。皮肤科情况:面颈部、双侧腋窝、躯干可见多发黄豆至蚕豆大小红褐色斑片,部分触之有浸润感。体检:系统检查无异常。实验室检查示高γ-球蛋白血症。皮损组织病理示:真皮全层血管及附属器周围见较多炎细胞浸润,以浆细胞为主。免疫组织化学示:浸润的浆细胞CD20、CD38、κ和λ轻链均阳性。诊断:皮肤浆细胞增生症。

A 53-year-old female presented with reddish-brown patches on the face,neck,axilla and trunk for 3 years.On physical examination,multiple reddish-brown patches of soybean to broadbean size were found on the face,neck,axilla and trunk,and some lesions had a sense of infiltration.No systemic manifestations were noted.Laboratory examination showed hyper γ globulinemia.Histopathological examination revealed perivascular and periadnexal infltration of inflammatory cells in the whole dermis,predominantly plasma cells.Immunohistochemistry displayed positive staining for CD20,CD38,κ and λ chain.The final diagnosis was cutaneous plasmacytosis.
