运动平板试验联合冠状动脉CT血管成像对冠心病诊断的对比研究 被引量:15

摘要 目的探讨运动平板试验(TET)联合冠状动脉CT血管成像(CCTA)对冠心病的诊断效能。方法以冠状动脉血管造影(CAG)为金标准,回顾性分析136例疑似冠心病患者的TET和CCTA资料,着重分析其TET、CCTA、TET联合CCTA的术前诊断效能。结果单独TET诊断冠心病的灵敏度、特异度、准确性、阳性预测值、阴性预测值分别为78. 95%、58. 33%、69. 85%、70. 58%、68. 63%。单独CCTA检出冠心病的灵敏度、特异度、准确性、阳性预测值、阴性预测值分别为86. 84%、63. 33%、76. 47%、75. 00%、79. 17%。TET联合CCTA诊断冠心病的灵敏度、特异度、准确性、阳性预测值、阴性预测值分别为93. 42%、78. 33%、86. 76%、84. 52%、90. 38%。TET联合CCTA诊断冠心病的特异度、准确性、阴性预测值均明显高于单纯TET和CCTA,其差异有统计学意义(P=0. 033,P=0. 003,P=0. 023),而诊断的灵敏度和阳性预测值差异无统计学意义(P=0. 054,P=0. 090)。TET、CCTA、TET联合CCTA一致性检验Kappa值分别为0. 45、0. 54、0. 77。结论 TET联合CCTA可提高冠心病的诊断效能。
