马来酸桂哌齐特联合单唾液酸四己糖神经节苷脂钠治疗急性脑梗死的效果及对血清扣针蛋白5和胆碱酯酶水平的影响 被引量:19

摘要 目的探讨马来酸桂哌齐特联合单唾液酸四己糖神经节苷脂钠治疗急性脑梗死的效果及对血清扣针蛋白5和胆碱酯酶水平的影响。方法选择2015年1月至2018年1月在江苏省海安市中医院就诊的急性脑梗死患者130例,应用随机数字表法分为观察组和对照组,各65例。对照组在常规治疗基础上给予马来酸桂哌齐特治疗,观察组在对照组基础上给予单唾液酸四己糖神经节苷脂钠治疗;比较2组患者的临床疗效以及治疗前后Barthel评分、血清胆碱酯酶和扣针蛋白5水平的变化。结果治疗2周后,观察组的总有效率高于对照组[89. 2%(58/65)比76. 9%(50/65)],差异有统计学意义(P <0. 05)。治疗前,2组患者的Barthel评分、血清胆碱酯酶和扣针蛋白5水平比较差异均无统计学意义(均P> 0. 05)。治疗2周后,2组患者Barthel评分和血清胆碱酯酶水平均高于治疗前,且观察组高于对照组[(76±7)分比(54±8)分,(4 784±226) U/L比(3 307±236) U/L]; 2组患者血清扣针蛋白5水平均低于治疗前,且观察组低于对照组[(70±31)μg/L比(90±32)μg/L],差异均有统计学意义(均P <0. 05)。结论马来酸桂哌齐特联合单唾液酸四己糖神经节苷脂钠治疗急性脑梗死患者的效果较好,可改善脑组织的缺氧缺血,从而降低血清扣针蛋白5水平,升高胆碱酯酶水平,改善预后,提高患者的生活能力。 Objective To investigate the effect of cinepazide maleate combined with monosialotetrahexosylganglioside sodium on serum fibulin-5 and cholinesterase in patients with acute cerebral infarction. Methods Totally 130 patients with acute cerebral infarction were enrolled from January 2015 to January 2018 in Haian Hospital of Traditional Chinese Medicine, Jiangsu Province. They were randomly divided into observation group and control group, with 65 cases in each group. On the basis of conventional therapy, the control group was treated with cinepazide maleate and the observation group was treated with monosialotetrahexosylganglioside sodium plus cinepazide maleate. Clinical efficacy, Barthel index, levels of serum cholinesterase and fibulin-5 were analyzed. Results After 2 weeks of treatment, the total effective rate in observation group was higher than that in control group [89.2%(58/65) vs 76.9%(50/65)](P<0.05). There was no significant difference of Barthel index, serum levels of cholinesterase and fibulin-5 between groups before treatment(P>0.05). After 2 weeks of treatment, Barthel index and serum cholinesterase level increased and showed significant differences between observation group and control group[(76±7) vs (54±8), (4 784±226)U/L vs (3 307±236)U/L];serum fibulin-5 level decreased after treatment and the level in observation group was lower than that in control group[(70±31)μg/L vs (90±32)μg/L](P<0.05). Conclusions Monosialotetrahexosylganglioside sodium combined with cinepazide maleate has good therapeutic effect on acute cerebral infarction. It can improve the hypoxic-ischemia of brain tissue, reduce serum fibulin-5, increase cholinesterase level and improve the prognosis and living ability.
