穴位注射牛痘疫苗致炎兔皮提取物配合针刺治疗腰椎间盘突出根性疼痛临床研究

Acupoint-Injected Vaccinia Virus for Inflammatory Rabbit Skin Extracts Combined with Acupuncture for the Treatment of Lumbar Intervertebral Disc Herniation

目的:观察穴位注射牛痘疫苗致炎兔皮提取物配合针刺治疗腰椎间盘突出根性疼痛的临床疗效。方法:选取64例腰椎间盘突出引起根性疼痛患者,采用随机数字表法分为观察组和对照组各32例。观察组患者采用穴位注射牛痘疫苗致炎兔皮提取物配合针刺治疗,对照组患者采用单纯针刺治疗,采用长海痛尺对两组患者疼痛程度进行评定,采用下腰痛JOA评分标准评定腰椎功能,并对其综合疗效进行评价。结果:经过治疗,观察组患者总有效率为93.55%,明显优于对照组的83.33%,差异有统计学意义(P<0.05)。两组患者治疗后及随访2个月的疼痛评分与治疗前比较均有所降低,差异均有统计学意义(P<0.05)。治疗后观察组患者的疼痛评分低于对照组,差异有统计学意义(P<0.05);且在随访2个月时观察组患者的疼痛评分仍低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后及随访2个月的JOA评分与治疗前比较均有所升高,差异均有统计学意义(P<0.05)。治疗后观察组患者的JOA评分高于对照组,差异有统计意义(P<0.05);且在随访2个月时观察组患者的JOA评分仍高于对照组,差异有统计学意义(P<0.05)。两组患者在试验过程中均未出现晕针、滞针、断针或皮下出血等不良事件,均未出现药疹、过敏反应等不良反应。结论:穴位注射牛痘疫苗致炎兔皮提取物结合针刺治疗腰椎间盘突出根性疼痛疗效优于单纯针刺治疗,且在镇痛、改善下腰痛JOA评分方面具有一定优势。

Objective:To observe the clinical effect of acupoint injection of extract of rabbit skin inflamed by vaccinia vaccine combined with acupuncture on root pain of lumbar intervertebral disc herniation.Methods:64 patients with radicular pain caused by lumbar disc herniation were randomly divided into observation group and control group,32 cases in each group.The patients in the observation group were treated with acupoint injection of extract of rabbit skin inflamed by vaccinia vaccine combined with acupuncture.The patients in the control group were treated with acupuncture alone.The pain degree of the patients in the two groups was evaluated by Changhai Pain Scale.The JOA score of low back pain was used to evaluate the lumbar vertebral function,and the comprehensive curative effect was compared and evaluated.Results:After treatment,the total effective rate of the observation group was93.55%,which was significantly better than 83.33% of the control group(P<0.05).The pain scores of the two groups after treatment and 2 months follow-up were lower than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the pain score of the patients in the observation group was lower than that of the control group(P<0.05),and the pain score of the patients in the observation group was still lower than that of the control group at 2 months of follow-up(P<0.05).The JOA scores of the two groups after treatment and 2 months follow-up were higher than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the JOA score of the patients in the observation group was higher than that of the control group,with statistical significance(P<0.05);and the JOA score of the patients in the observation group was still higher than that of the control group at 2 months of follow-up,with statistical significance(P<0.05).There were no adverse events such as fainting,staying,breaking or subcutaneous hemorrhage,and no adverse reactions such as drug eruption and allergic reaction.Conclusion:Acupoint injection of vaccinia vaccine-induced inflammatory rabbit skin extract combined with acupuncture is superior to simple acupuncture in the treatment of lumbar intervertebral disc herniation and root pain,and has relative advantages in analgesia and improving JOA score of low back pain.
