摘要 急性肾损伤(acute kidney injury,AKI)明显影响患者的临床预后,其发病机制不清,且临床缺乏有效的治疗手段,缺少可早期预测AKI发生的标志物。肌酐作为目前诊断手段既缺乏敏感度,也缺少特异度。临床迫切需要能够早期准确地预测肾功能损伤的生物学标志物。最近的研究表明,细胞周期停滞可能与AKI的发生密切相关。细胞周期停滞时产生的尿基质金属蛋白酶(TIMP-2)和胰岛素样生长因子结合蛋白(IGFBP-7)可在早期准确预测AKI的发生。 Acute kidney injury (AKI)is common in critically ill patients associated with increased morbidity and mortality without effective treatment.The mechanism of AKI is still largely unknown.Prediction of AKI in the early phase is difficult.Serum creatinine,the most widely used marker of renal function,has been reported neither sensitive nor sepcific.Recently,investigations have shown that cell cycle arrest may play an important role in the development of AKI and biomarkers of cell cycle arrest TIMP -2 and IGFBP -7 may accurately predict early AKI.
