以医联体为依托的居家护理对直肠癌出院患者自我护理能力及心理-社会功能的影响

Effects of home care based on regional medical union on self-care agency and psychosocial function among rectal cancer discharged patients

目的探讨以医联体为依托的居家护理干预对直肠癌患者自我护理能力、心理-社会功能的影响。方法于2016年8月将70例接受直肠癌术后的患者分为对照组和观察组,每组35例。对照组采取常规门诊随访,观察组采用以医联体为依托的居家护理服务模式。比较两组患者干预后的自我护理能力(自我护理技能、自护责任感、自我概念、健康知识水平)及心理-社会功能(焦虑评分、抑郁评分、Herth希望水平、社会支持度)。结果观察组的自我护理技能、自我责任感、自我概念、健康知识水平各项得分及总分分别为(28.40±1.68)、(17.35±2.21)、(21.75±1.25)、(47.02±2.53)、(114.52±6.54)分,均较对照组高,差异具有统计学意义(P<0.05)。观察组的支持利用度、客观社会支持度和主观社会支持度分别为(10.64±1.42)、(13.74±0.83)、(25.27±1.25)分,高于对照组;抑郁和焦虑评分分别为(42.45±1.85)、(32.06±1.18)分,低于对照组;希望水平为(41.43±3.25)分,高于对照组;差异均具有统计学意义(P<0.05)。结论以医联体为依托的居家护理模式能够增强直肠癌患者的自我护理能力,改善患者的心理-社会功能。

Objective To explore the effects of home care based on regional medical union on self-care agency and psychosocial function among rectal cancer patients. Methods In August 2016, a total of 70 patients underwent surgery for rectal cancer were divided into control group and observation group, 35 cases in each group. Control group received routine outpatient follow-up, while observation group carried out home nursing care based on regional medical union. And then, we compared the self-care agency (self-care skills, self-care responsibility, self-concept, health knowledge) and psychosocial function (anxiety, depression, Herth hope index, social support) . Results The scores of self-care skills, self-care responsibility, self-concept as well as health knowledge and the total score in observation group were (28.40±1.68) , (17.35±2.21) , (21.75±1.25) , (47.02±2.53) and (114.52±6.54) higher than those in control group with significant differences (P<0.05) . The scores of availability of social support, objective social support and subjective social support in observation group were (10.64±1.42) , (13.74±0.83) and (25.27±1.25) , higher than those in control group;the scores of depression and anxiety in observation group were (42.45±1.85) and (32.06±1.18) , lower than those in control group;the Herth hope index in observation group was (41.43±3.25) , higher than that in control group;the differences were statistically significant (P<0.05) . Conclusions Home care based on regional medical union can improve self-care agency and psychosocial function among rectal cancer patients.
