上海某社区骨质疏松症患者用药依从性影响因素研究分析

Influencing factors of drug compliance of osteoporosis patients in a community of Shanghai

目的:调查骨质疏松症患者用药依从性,分析可能的影响因素,对后期干预给予参考。方法:采用自制问卷对198名社区骨质疏松症患者进行用药依从性调查。结果:单因素分析显示:患者的年收入高(χ2=7. 762,P=0. 021)、年龄相对较小(χ2=27. 8,P=0. 000)、文化程度高(χ2=27. 904,P=0. 000)、对疾病认知水平高(χ2=12. 429,P=0. 002);无不良反应(χ2=38. 088,P=0. 000)、无其他疾病用药情况(χ2=30. 543,P=0. 000)、治疗效果好(χ2=8. 928,P=0. 012)、药物价格接受程度高(χ2=7. 180,P=0. 028)和骨质流失严重(χ2=41. 113,P=0. 000)用药依从性较好;多因素Logistic回归分析发现,可变因素中治疗效果好(OR=3. 719,P=0. 000)和骨质疏松知识认知得分高(OR=2. 617,P=0. 000);不变因素中,文化程度高(OR=2. 417,P=0. 003)和年龄相对较小(OR=1. 831,P=0. 041)对用药依从性有积极影响。结论:骨质疏松症患者用药依从性状况不容乐观,影响因素众多,建议针对主要因素开展干预教育。

Objective To investigate the drug compliance of osteoporosis patients,to analyze the possible influencing factors,and to provide reference for later intervention. Methods A total of 198 community osteoporosis patients were surveyed by selfdesigned questionnaires. Results Univariate analysis showed that high annual income( χ^2= 7. 762,P = 0. 021),younger age( χ^2= 27. 8,P = 0. 000),better education level( χ^2= 27. 904,P = 0. 000),good disease awareness( χ^2= 12. 429,P =0. 002),no adverse reactions( χ^2= 38. 088,P = 0. 000),no medication for other diseases( χ^2= 30. 543,P = 0. 000),good therapeutic effect( χ^2= 8. 928,P = 0. 012),better acceptance of drug price( χ^2= 7. 180,P = 0. 028) and severity of bone loss( χ^2= 41. 113,P = 0. 000) all had positive effects on medication compliance. Logistic regression analysis showed that variablesincludinggood therapeutic effect( OR = 3. 719,P = 0. 000) and better knowledge of osteoporosis( OR = 2. 617,P =0. 000) and invariant factors including good education( OR = 2. 417,P = 0. 003) and younger age( OR = 1. 831,P = 0. 041)had a positive impact on drug compliance. Conclusions Drug compliance of osteoporosis patients is not good,which has been influenced by various factors. It is suggested that comprehensive intervention programs be carried out for main influencing factors.
