血清中DKK1表达在食管鳞癌诊断中的应用价值

The application value of serum DKK1 in the diagnose of esophageal squamous cell carcinoma

[目的]检测DKK1血清中表达在食管鳞癌患者诊断中的应用价值。[方法]应用酶联免疫检测法(ELISA)检测76例食管鳞癌患者和76例正常人血清中DKK1和CEA的表达水平,并将两者进行比较,探讨其在食管鳞癌诊断中的应用价值。[结果]食管鳞癌患者血清中DKK1浓度为(108.27±27.58)ng/ml,正常人血清浓度为(92.25±20.69)ng/ml,两者差异有统计学意义(P<0.000)。拟合ROC曲线,计算食管鳞癌患者血清中DKK1的灵敏度是57.89%,特异度是89.47%。统计学分析显示,食管鳞癌患者血清中的DKK1水平与年龄、性别、病理分级、TNM分期、肿瘤浸润深度及淋巴结转移与否均无显著相关性(P>0.05)。传统肿瘤标志物CEA在食管鳞癌患者血清中浓度为(11.07±10.30)ng/ml,对照组为(5.08±4.42)ng/ml,两者差异有统计学意义(P<0.05)。拟合ROC曲线,计算食管鳞癌患者血清中CEA的诊断灵敏度是61.8%,特异度是68.4%。DKK1在食管鳞癌诊断中的特异度显著高于CEA。[结论]DKK1可作为检测食管鳞癌的一种新的肿瘤标志物,在食管鳞癌诊断中有一定的应用价值。

[Objective]To explore the expression of DKK1 in the serum of esophageal squamous cell carcinoma(ESCC)and its clinical significance.[Methods]Serum DKK1 and CEA were determined by sandwich enzyme linked immunosorbent assay(ELISA)in 76 patients with ESCC and in 76 healthy subjects.Their application value in the diagnosis of ESCC was also explored.[Results]Serum DKK1 in ESCC patients and healthy subjects were(108.27±27.58)ng/ml and(92.25±20.69)ng/ml,respectively.The difference was statistically significant(P<0.000).Fitting the ROC curve,serum DKK1 of patients with esophageal squamous cell carcinoma,its sensitivity and specificity were 57.89% and 89.47%,respectively.Statistical analysis showed that there was no significant difference between serum DKK1 and age,sex,pathology grade,TNM staging,tumor invasive depth and lymphatic metastasis of ESCC patients(P >0.05).Traditional tumor marker serum CEA in ESCC patients and healthy subjects were(11.07±10.30)ng/ml and(5.08±4.42)ng/ml,respectively.The difference was statistically significant(P<0.000).Fitting the ROC curve,serum CEA of patients with esophageal squamous cell carcinoma,its sensitivity and specificity were 61.8%and 68.4%,respectively.The specificity of ESCC of DKK1 is higher than that of CEA in the diagnosis of esophageal squamous cell carcinoma.[Conclusion]DKK1 has certain application value in the diagnose of ESCC and could be used as a novel tumor marker in the diagnose of E SCC.
