肺癌脑转移瘤多发患者开放手术治疗的效果及生存分析

Effect and survival analysis of open surgery in patients with multiple brain metastases of lung cancer

目的分析肺癌脑转移瘤多发患者开放手术治疗的效果及生存情况。方法将2013年4月至2014年3月我院收治的72例肺癌脑转移瘤多发患者纳入本次研究。患者在完成脑转移瘤切除术后行全脑放疗、化疗,有40例患者行肺部肿瘤切除术,32例未开展肺部肿瘤切除术。分析患者生存期和生存率。结果所有患者的生存期为7. 12～53. 14个月,中位生存期为(24. 32±5. 32)个月,1年、2年、3年生存率分别为83. 33%(60/72)、48. 61%(35/72)、16. 67(12/72)。接受肺叶切除术和未接受肺叶切除术者的生存时间分别是(21. 45±2. 75)个月、(22. 38±2. 81)个月,无显著差异(P> 0. 05),接受肺叶切除术者的1年、2年、3年生存率分别为90. 00%(36/40)、50. 00%(20/40)、17. 50%(7/40),未接受肺叶切除术者的1年、2年、3年生存率分别为75. 00%(24/32)、46. 88%(15/32)、15. 63%(5/32),无明显差异(P> 0. 05)。结论肺癌脑转移瘤多发患者通过开放手术切除脑转移瘤,效果良好,是否切除肺部肿瘤并不会影响患者生存期。

Objective To analyze the effect and survival of open surgery in patients with multiple brain metastases of lung cancer.Methods From April 2013 to March 2014,72 patients with multiple brain metastases of lung cancer admitted to our hospital were included in this study.Patients undergoing brain metastases resection surgery after whole brain radiotherapy and chemotherapy,40 patients underwent lung tumor resection,32 patients underwent lung tumor resection.Analysis of patient survival time and survival rate.Results The survival rates of all pa- tients ranged from 7.12 to 53.14 months.The median survival time was (24.32±5.32)months. The overall survival rates were 83.33%(60/72),48.61%(35/72),16.67 (12/72).The survival time of those who received lobectomy and those who did not undergo lobectomy respectively was (21.45±2.75)months,and (22.38±2.81)months,with no significant difference (P>0.05). The 1 year,2 year and 3 year survival rate of those who underwent lobectomy re spectively was 90.00%(36/40),50.00%(20/40)and 17.50 %(7/40),The 1 year,2 year and 3 year survival rate of patients without lobectomy respectively was 75.00%(24/32),46.88%(15/32),15.63% (5/32),with no significant difference (P>0.05).Conclusion Multiple brain metastases of lung cancer patients with brain metastases through open surgery,the effect is good,whether the removal of lung cancer does not affect the survival of patients.
