摘要
目的探讨脑血管介入术治疗脑动脉粥样硬化的临床疗效及对远期复发率的影响。方法选取本院收治的67例脑动脉粥样硬化患者作为研究对象,使用随机数字表法分为两组,对照组(32例)接受药物治疗,观察组(35例)行脑血管介入术。对比两组患者治疗前后的纤溶活性、颈动脉血流、生活质量变化及远期复发情况。结果观察组治疗后的tPA、vWF水平高于对照组,PAI-1水平低于对照组(P <0. 05)。观察组治疗后的基底动脉、椎动脉、颈总动脉、颈内动脉的Vs值均低于对照组(P <0. 05)。观察组治疗后在躯体功能、心理功能、社会功能、物质生活方面的生活质量评分均高于对照组,治疗后1年的复发率、病死率均低于对照组(P <0. 05)。结论脑动脉粥样硬化患者行脑血管介入术能够有效改善血管的纤溶活性及颈动脉血液流速,提高患者的生活质量,并且其远期复发率较低,患者预后效果好。
Objective To explore the efficacy of cerebrovascular interventional in the treatment of cerebral atherosclerosis and the influences on long-term recurrence rate. Methods 67 patients with cerebral atherosclerosis were divided into two groups by random number table method. The control group( 32 cases) received drug treatment. The observation group( 35 cases) received cerebrovascular intervention. Then compared the fibrinolytic activity,carotid blood flow,quality of life changes and long-term recurrence before and after treatment between two groups. Results The levels of t PA and v WF in the observation group after treatment were higher than control group,and the levels of PAI-1 were lower than control group( P < 0. 05). The Vs values of basilar artery,vertebral artery,common carotid artery and internal carotid artery of the observation group after treatment were lower than control group( P < 0. 05). The quality of life of observation group after treatment in physical function,psychological function,social function and physical life were higher than control group,and the recurrence rate and mortality after treatment 1 year were lower than control group( P < 0. 05). Conclusion Cerebrovascular intervention for patients with cerebral atherosclerosis can effectively improve the fibrinolytic activity of blood vessels and carotid blood flow velocity and improve the quality of life of patients,and its long-term recurrence rate is low,the prognosis of patients with good effect.
