腓动脉穿支皮瓣修复足踝部皮肤软组织缺损

Repair of foot or ankle soft tissue defects by peroneal artery perforator flap

目的探讨腓动脉穿支皮瓣修复足踝部皮肤软组织缺损的手术方法和临床疗效。方法 2011年12月-2017年3月,对20例足踝部创面采用三种腓动脉穿支皮瓣进行修复,其中腓动脉外踝后穿支皮瓣1例,腓动脉外踝后上穿支皮瓣14例,腓动脉外踝前上穿支皮瓣5例。结果 18例皮瓣完全成活,1例表皮坏死,1例远端坏死,经换药后均愈合。20例获得3～68个月的随访,平均24.1个月,修复后足踝部功能良好,外形满意。结论腓动脉穿支皮瓣是修复足踝部皮肤软组织缺损的一种良好方法。

Objective To discuss the surgical techniques and clinic result of peroneal artery flap for reconstruction of foot or ankle soft tissue defects.Methods From December 2011 to March 2017,20 cases of foot or ankle soft tissue defects were involved.Peroneal artery lateral retromalleolar perforator flap was used in 1 case,peroneal artery postero-lateral suprarnalleolar perforator flap were used in 14 cases,peroneal artery antero-lateral supramalleolar perforator flap were used in 5 cases.Results Eighteen flaps survived.1 flap had epidermal necrosis,1 flap had distal necrosis,and the wounds closed after change dressings many times.20 patients were followed-up for 3 to 68 months(mean 24.1 months).No patients lost follow-up.The functional and cosmetic outcomes of foot and ankle were excellent.Conclusion Peroneal artery perforator flap is a good satisfactory way for reconstruction of foot or ankle tissue defects.
