血必净注射液对有机磷中毒并呼吸衰竭患者的治疗效果 被引量:2

摘要 目的:探讨血必净注射液对有机磷中毒并呼吸衰竭患者的治疗效果。方法:选取2015年1月~2018年6月我院重症监护病房(ICU)诊断为重度有机磷中毒并需要机械通气的60例患者作为研究对象,随机分为对照组和治疗组,每组30例。对照组接受一般综合治疗,治疗组在对照组的基础上,采取静脉滴注血必净注射液。治疗第7天,比较两组患者的氧合指数(OI)、白细胞(WBC)、乳酸(Lac)、APACHEⅡ评分、胆碱酯酶(ChE)活性恢复时间、呼吸机上机时间和多重耐药菌(MDR)发生率等指标。结果:治疗第7天,治疗组的OI明显高于对照组,WBC和Lac明显低于对照组,差异均有统计学意义,P<0.05;治疗组的APACHEⅡ评分、ChE恢复时间、呼吸机上机时间和多重耐药菌发生率均明显低于对照组,差异均有统计学意义,P<0.05。结论:血必净注射液能够明显改善有机磷中毒并呼吸衰竭患者的氧合指数,降低其炎症反应,缩短胆碱酯酶活性恢复时间,减少多重耐药菌发生率,提高抢救成功率。 Objective: To explore the clinical effect of Xuebijing injection on patients with organophosphorus pesticide poisoning and respiratory failure. Methods: From January 2015 to June 2018, 60 patients diagnosed as severe organophosphorus pesticide poisoning and also needed mechanical ventilation were randomly divided into control group and treatment group, with 30 patients in each group. The control group was given conventional comprehensive treatment. The treatment group was given intravenous infusion of Xuebijing injection on the basis of the control group. On the 7 th day of treatment, compared the oxygenation index(OI), white blood cell(WBC)level, lactic acid(Lac) level, APACHE Ⅱ scores, cholinesterase(ChE) activity recovery time, ventilator time and incidence of multidrug-resistant bacteria(MDR) of the two groups. Results: On the 7 th day of the treatment, the OI of the treatment group was significantly higher than that of the control group, the level of WBC and Lac of the treatment group were significantly higher than that of the control group, the differences were statistically significant, P<0.05;the APACHE Ⅱ score, the time of Ch E recovery, ventilator time and MDR of the treatment group were significantly lower than that of the control group, the difference were also statistically significant,P<0.05. Conclusion: Xuebijing injection can improve oxygenation index in patients with organophosphorus pesticide poisoning and respiratory failure, reduce their inflammation, shorten the cholinesterase activity recovery time, decrease the incidence of multi-resistant bacteria, and improve the success rate of rescue.
