中性粒细胞/淋巴细胞比值与冠心病患者左心室肥厚的相关性分析

Relationship between neutrophil to lymphocyte ratio and left ventricular hypertrophy in patients with coronary heart disease

目的探讨中性粒细胞/淋巴细胞比值(NLR)与冠心病患者左心室肥厚(LVH)之间的关系。方法选取200例经冠脉造影确诊为冠心病的患者,根据左心室质量指数(LVMI)分为左心室肥厚组(88例)和左心室正常组(112例),收集空腹血液样本的用于血常规、血脂、血糖、胰岛素等检测,并对所有冠心病患者行心脏超声检查。结果在冠心病患者中,左心室肥厚组收缩压、BMI、NC、NLR、FBG、FINS、HOMA-IR和LVMI均高于左心室正常组,而淋巴细胞计数(LC)低于左心室正常组,且差异均有统计学意义(P<0.05),2组患者年龄、舒张压、TC、TG、LDL-C和HDL-C差异无统计学意义(P>0.05);Pearson相关分析提示,在冠心病患者中,LVMI与NLR呈正相关(r=0.319,P<0.001);校正收缩压、BMI、HOMA-IR等危险因素后, NLR仍然是冠心病患者发生LVH的独立危险因素,OR值为2.876(95%CI:2.001～4.245,P<0.05)。结论冠心病合并LVH者NLR升高,NLR升高可能是冠心病患者发生LVH的独立危险因素。

Objective To explore the relationship between neutrophil to lymphocyte ratio(NLR) and left ventricular hypertrophy(LVH) in coronary heart disease. Methods A total of 200 patients with coronary heart disease were divided into two groups according to left ventricular myocardial mass index(LVMI): LVH group(n=88) and non-LVH group(n=112). Blood samples were collected for the detection of blood routine, blood lipids, blood glucose, insulin, and all patients with coronary heart disease underwent echocardiography. Results Compared with non-LVH group, the LVH group had higher systolic blood pressure, BMI, NC, NLR, FBG, FINS, HOMA-IR and LVMI, while it had lower LC, with the differences statistically significant(P<0.05). There was no statistical significance on the differences of age, diastolic blood pressure, TC, TG, HDL-C and LDL-C between the two groups(P>0.05). Pearson correlation analysis showed that LVMI was positively correlated with NLR(r=0.319, P<0.001). NLR was an independent risk factor for LVH in patients with coronary heart disease with after adjusted for systolic blood pressure, BMI, HOMA-IR and so on [OR=2.876(95%CI: 2.001-4.245), P<0.05]. Conclusion Patients with coronary heart disease have higher NLR and NLR, which may be an independent risk factor for patients with coronary heart disease and LVH.
