自体耳软骨双层SEG联合下外侧软骨外侧脚缝合在初次鼻整形鼻尖延长中的效果观察

Effect of autologous auricular cartilage bilayer septal extension graft(SEG) combined with lateral feet suture of lower lateral cartilage for nasal tip extension in primary rhinoplasty

目的观察自体耳软骨制成双层鼻中隔延伸移植物(septal extension graft, SEG),联合下外侧软骨外侧脚缝合用于鼻尖延长的效果。方法选取有短鼻延长和隆鼻要求的初次鼻整形患者29例,取耳甲艇和耳甲腔软骨。其中耳甲艇软骨用于制作双层SEG,同时联合下外侧软骨外侧脚缝合延长鼻尖;耳甲腔软骨用于制作帽状移植物进行鼻尖成形;鼻背抬高采用硅胶假体或膨体。结果本组29例患者术后随访12个月,均获得良好的鼻尖延长效果,无明显鼻尖下降。结论自体耳软骨双片SEG联合下外侧软骨外侧脚缝合用于初次鼻整形的鼻尖延长,可获得满意的效果。

Objective To observe the effect of bilayer nasal septal extension graft(SEG) made of autologous auricular cartilage combined with lateral feet suture of lower lateral cartilage for nasal tip extension. Methods Twenty-nine patients with congenital short noses were selected. In each case, the cymba conchae and auricular cartilage were collected and the cymba conchae was used to make a bilayer SEG. The nasal tip was extended through SEG combined with lateral feet suture of the lower lateral cartilage. The auricular cartilage was used to make a cap graft or shield graft for nasal tip shaping. The silicon or e-PTFE was implanted to elevate the nasal dorsum.Results All patients were followed-up postoperatively for 12 months and good results were achieved in the nasal tip extension. No obvious tip retraction or dropping was found. Conclusion Surgeons can obtain satisfactory results through the application of autologous auricular cartilage bilayer septal extension graft(SEG) combined with lateral feet suture of the lower lateral cartilage for nasal tip extension in primary rhinoplasty.
