血清25-(OH)维生素D水平与进展期胃癌新辅助化疗后生存时间的相关性研究 被引量:4

Correlations between serum 25(OH) vitamin D levels and the prognosis of advanced gastric cancer patients with neoadjuvant chemotherapy

摘要 目的:探讨血清25(OH)维生素D水平与新辅助化疗后进展期胃癌病人预后的相关性。方法:选择解放军南京总医院2007年1月至2010年1月间进行新辅助化疗的进展期胃癌病人167例,分析其血清25(OH)维生素D水平与其生存时间及其它临床病理学指标之间的关系。结果:病人平均血清25(OH)维生素D水平为(18.94±9.47)ng/mL。维生素D水平与治疗前临床淋巴结分期(cN)(P=0.004)、治疗后病理学分期(ypStage)(P=0.009)、治疗后原发灶分期(ypT)(P=0.002)等指标相关。血清25(OH)维生素D水平较高(vitamin D> 20 ng/mL)的病人的中位生存时间长于25(OH)维生素D水平较低(vitamin D≤20 ng/mL)的病人。多因素生存分析显示血清25(OH)维生素D水平是独立预后影响因素。结论:血清25(OH)维生素D水平与新辅助化疗后进展期胃癌病人的预后相关。 Objective: To investigate the association between the serum 25(OH) vitamin D levels and the prognosis of advanced gastric cancer patients with neoadjuvant chemotherapy. Methods: 167 advanced gastric cancer patients who received neoadjuvant chemotherapy between January 2007 and January 2010 were involved in this study.The correlation between 25(OH) vitamin D levels, overall survival time and other clinicopathological data was examined. Results: The mean 25(OH) vitamin D level of these patients was(18.94 ± 9.47) ng/mL. Vitamin D levels were correlated with clinical N stage(cN) before chemotherapy(P = 0.004), overall pathological stage after chemotherapy(ypStage)(P = 0.009), pathological T stage after chemotherapy(ypT)(P = 0.002). The patients with higher vitamin D levels(vitamin D > 20 ng/mL) had a better overall survival than those with lower vitamin D levels(vitamin D ≤ 20 ng/mL). Multivariate analysis indicated that vitamin D level was an independent prognostic factor for advanced gastric cancer patients undergoing neoadjuvant chemotherapy. Conclusion: Serum 25(OH) vitamin D levels were correlated with the prognosis of advanced gastric cancer patients undergoing neoadjuvant chemotherapy.
