临床药师参与的脑出血患者持续发热的病例分析

Analysis of persistent fever in patient with cerebral hemorrhage involving a clinical pharmacist

脑出血在临床诊断及治疗中比较常见的一种疾病,脑出血患者常合并存在发热症状,引发其脑血流量加大,提升脑氧代谢情况,提高脑出血患者的颅内压,还可能会损害其神经元,对脑出血患者的大脑带来一定损伤,对脑出血患者的机体健康带来严重危害,且威胁脑出血患者的生命安全。本文通过对1例脑出血后持续发热的重症患者治疗用药过程的回顾性分析,此例患者2018年1月19日转入重症监护室,入院后给予抗感染、护胃、祛痰、止咳、平喘、护脑、降压、肠内营养、脑室引流等对症治疗,患者在2018年1月20日开始出现发热,持续给予物理降温处理,直到2018年1月28日其体温才恢复正常。本文主要探讨脑出血患者持续发热的可能原因及药物的选择问题,分析临床药师参与到持续发热的脑出血患者临床治疗中的效果和价值。

Cerebral hemorrhage is a common disease in clinical diagnosis and treatment.Patients with cerebral hemorrhage often have symptoms of fever,causing increased cerebral blood flow,cerebral oxygen metabolism,and intracranial pressure.In addition,it possibly damages their neurons,which causes certain damage to the brain,poses serious harm to the health andthreatens the life safety of patients with cerebral hemorrhage.In this paper,the medication and treatment process of a critically ill patient with fever after cerebral hemorrhage was retrospectively analyzed.This patient was transferred to the intensive care unit on January 19,2018.After admission,he was given symptomatic treatments such as anti-infection,stomach protection,expellingphlegm,relieving cough,relieving asthma,brain protection,blood pressure reduction,enteral nutrition and ventricular drainage.The patient began to have fever on January 20,2018 and continued to receive physical cooling treatment until January 28,2018 when his body temperature returned to normal.This paper mainly discusses the possible causes of persistent fever in patients with cerebral hemorrhage and the problem of drug selection, and analyzes the effect and value of clinical pharmacists’ participation in the clinical treatment of persistent feverof patients with cerebral hemorrhage.
