对腰椎小关节综合征行体外冲击波联合椎间孔注射治疗的效果观察

Effect of extracorporeal shock wave combined with intervertebral foramen injection on lumbar facet joint syndrome

目的:探讨对腰椎小关节综合征行椎间孔注射联合体外冲击波治疗的效果。方法:选取140例我院收治的腰椎小关节综合征患者,入院时间为2014年3月至2018年1月,随机分为对照组(n=70,采取非甾体抗炎镇痛药治疗+体外冲击波治疗)与观察组(n=70,采取非甾体抗炎镇痛药治疗+体外冲击波治疗+椎间孔注射),观察相关指标及疗效。结果:观察组的治疗优良率显著高于对照组(P <0. 05);两组在治疗后各个时间段VAS评分、RMDQ评分、ODI评分均显著降低,观察组在治疗1、4、8周时上述评分均明显低于对照组(P <0. 05),两组治疗后QOL-SF36评分显著上升,且观察组的QOL-SF36评分显著高于照组(P <0. 05),此外,观察组患者住院时间明显短于对照组(P <0. 05)。结论:对腰椎小关节综合征行椎间孔注射及体外冲击波联合治疗的效果确切,能有效缩短病程,改善生活质量,具有临床推广价值。

Objective: To explore the effect of intervertebral foramen injection combined with extracorporeal shock wave therapy for lumbar facet joint syndrome. Methods: 140 patients with lumbar facet syndrome received from our hospital from March 2014 to January 2018 were randomly divided into control group( n = 70,nonsteroidal antiinflammatory analgesics therapy and extracorporeal shock wave therapy) and observation group( n = 70,nonsteroidal anti-inflammatory analgesic therapy + extracorporeal shock wave therapy + intervertebral foramen injection),the related index and effect were observed. Results: Compared with the control group the excellent and good rate of treatment in the observation group was significantly higher( P < 0. 05). After treatment,the VAS score,RMDQ score and ODI score in each time period were significantly decreased in the two groups,the scores in the observation group were significantly lower than those in the control group when they were treated for 1,4 and 8 weeks respectively( P <0. 05). The QOL-SF36 score increased significantly in the two groups,while the QOL-SF36 score in the observation group was significantly higher than that in the control group( P < 0. 05),in addition,the hospitalization time in the observation group was significantly shorter than that in the control group( P < 0. 05). Conclusion: The combination of intervertebral foramen injection and extracorporeal shock wave therapy for the lumbar facet syndrome is effective. It can effectively shorten the course of the disease and improve the quality of life. It has the value of clinical promotion.
