冠心病患者血清甘油三酯水平与左室射血分数的关系

Relationship between Serum Triglyceride Level and Left Ventricular Ejection Fraction in Coronary Heart Disease

目的探讨冠心病患者血清甘油三酯水平与左室射血分数的关系。方法连续入选2017年9月至2018年2月在西安交通大学第一附属医院进行住院治疗的冠心病患者共753例。收集患者的一般临床资料,并测定其血清甘油三酯水平和左室射血分数。采用单因素及多因素回归分析方法分析冠心病患者血清甘油三酯水平与左室射血分数的关系。结果左室射血分数(LVEF)≥50%组冠心病患者血清甘油三酯水平显著高于LVEF <50%的患者[(1.76±1.15mmol/L vs.(1.52±1.08) mmol/L,P=0.030)]。多因素回归分析结果显示高密度脂蛋白胆固醇(B=0.11,P=0.021)、甘油三酯水平(B=0.11,P=0.013)与LVEF呈正相关,而心率(B=-0.27,P <0.001)与LVEF呈负相关。结论冠心病患者血清甘油三酯随着左室射血分数的降低而降低,未来仍需多中心研究进一步评估中国人群血清甘油三酯水平与冠心病预后的关系。

Objective To investigate the relationship between serum triglyceride level and left ventricular ejection fraction in coronary heart disease(CHD). Methods 753 cases of coronary heart disease diagnosed by coronary angiography in the First Affiliated Hospital of Xi’an Jiaotong University from September 2017 to February 2018 were selected consecutively. The serum triglyceride concentration was detected by the Hitachi automatic biochemical analyzer and the left ventricular ejection fraction(LVEF) was measured by echocardiography. The relationship between serum triglyceride and left ventricular ejection fraction in coronary heart disease was analyzed by univariate and multivariate regression analysis. Results The expression of serum triglyceride in patients with LVEF≥50% were significantly higher than those in patients with LVEF<50% [1.76±1.15] mmol/L vs. [1.52±1.08] mmol/L, P=0.030). Multivariate regression analysis showed that high density lipoprotein cholesterol(B =0.11, P =0.021) and triglyceride(B=0.11, P =0.013) were positively correlated with LVEF, while heart rate(B=-0.27, P <0.001) was negatively correlated with LVEF. Conclusions The levels of serum triglyceride in patients with coronary heart disease decrease with the decrease of left ventricular ejection fraction. Future multi-center studies should assess the relationship between serum triglyceride levels and the prognosis of coronary heart disease.
