急性ST段抬高型心肌梗死患者院内死亡的危险因素

Risk Factors Analysis for Death During Hospital in Patients with Acute ST-segment Elevation Myocardial Infarction

目的探讨急性ST段抬高型心肌梗死患者院内死亡的相关危险因素。方法收集增城区人民医院2015年2月至2018年2月住院的416例急性ST段抬高型心肌梗死患者的临床资料,根据患者是否在院内死亡分为生存组(385例)和死亡组(31例),采用单因素和多因素logistic回归分析患者院内死亡的相关危险因素。结果 416例患者院内死亡发生率为7.45%,男性22例,占比5.29%;女性9例,占比2.16%。泵功能衰竭(包括心源性休克)、心脏破裂是院内死亡的两大主要原因。多因素logistic回归分析显示年龄、发病时间、心率、左主干或3支血管病变、随机血糖≥11 mmol/L是急性心肌梗死院内死亡的独立危险因素。结论高龄、发病时间长、入院时心率快、左主干或3支血管病变、随机血糖≥11 mmol/L是急性心肌梗死院内死亡的独立危险因素。

Objective To analyze the risk factors of in-hospital death in patients with acute ST-segment elevation myocardial infarction.Methods Clinical data of 416 patients with acute ST-segment elevation myocardial infarction were collected from Feb.2015 to Feb.2018 in ZengCheng people's hospital. Patients were divided into survival group(385 cases)and death group(31 cases)according to whether they died in hospital.Single and muhi-variable Logistic regression analysis were used to analyze the risk factors of in-hospital mortality.Results The overall mortality rate of 416 patients in hospital was 7.45 %,including 22 males,accounting for 5.29%.There were 9 females,accounting for 2.16%.Pump failure (including card!ogenic shock)and cardiac rupture were the two leading causes of in-hospital death in patients with acute ST-segment elevation myocardial infarction.Multi-variable logistic regression analysis revealed that the age, onset time,heart rate,left main or 3 vascular lesions,random blood glucose above 11.1 mmoL/L were independent risk factors for in-hospital death in acute ST-segment elevation myocardial infarction.Conclusion Elderly age,long onset time,rapid heart rate,left main or 3 vascular lesions,random blood glucose above 11.1 mmol/L maybe independent risk factors for in-hospital death in patients with acute ST-segment elevation myocardial infarction.
