三磷酸腺苷酶家族蛋白2在肝门部胆管癌中的表达及其临床意义

Expression of ATAD2 in hilar cholangiocarcinoma and the clinical significance

目的探讨三磷酸腺苷酶家族蛋白2(ATAD2)在肝门部胆管癌(hilar cholangiocarcinoma, HC)中的表达水平及其与HC患者临床病理因素和预后的关系。方法采用免疫组化、qRT-PCR及Western blot检测ATAD2在HC的表达情况,分析ATAD2的表达水平与临床病理因素和预后的关系。结果HC组织中ATAD2阳性表达率70%(57/81),显著高于正常胆管组织的7%(2/30),差异有统计学意义(χ2=35.678,P<0.01),且ATAD2表达与淋巴结转移、TNM分期有关(分别χ2=28.619,31.612,均P<0.01)。ATAD2阴性HC患者术后1、3和5年生存率分别为87.5%、54.1%和33.3%,ATAD2阳性HC患者术后1、3和5年生存率分别为40.3%、14.0%和1.7%,两组患者生存率比较差异有统计学意义(χ2=14.162,P<0.01)。单因素分析与多因素COX回归模型分析结果显示,微血管侵犯、淋巴结转移以及ATAD2表达是HC患者预后的独立影响因素(分别F=4.703,4.961,5.013,均P<0.05)。结论ATAD2在HC中表达增加,且ATAD2过表达与HC的发生、发展和侵袭转移及不良预后密切相关。

Objective To investigate the expression and clinical significance of ATAD2 in patients with hilar cholangiocarcinoma (HC). Methods The qRT-PCR,Western blottig and immunohistochemistry were used to detect the expression of ATAD2 in HC and normal bile duct tissues,and the relationship between ATAD2 and the clinicopathological factors,prognosis was analyzed. Results The positive expression rate of ATAD2 in HC tissues was 70% (57/81),significantly higher than that of normal bile duct tissues 7%(2/30) (χ2=35.678,P<0.01),and ATAD2 expression was associated with lymph node metastasis and TNM staging (respectively χ2=28.619,31.612,all P<0.01).The 1′-,3′-,and 5-year survival rates of ATAD2 negative HC patients were 87.5%,54.1%,and 33.3%,respectively.The 1′-,3′-,and 5-year survival rates of ATAD2 positive HC patients were 40.3%,14.0% and 1.7%,respectively (χ2=14.162,P<0.01).Univariate analysis and multivariate COX regression model analysis showed that microvascular invasion,lymph node metastasis and ATAD2 expression were independent factors influencing the prognosis of patients with HC (respectively F=4.703,4.961,5.013,all P<0.05). Conclusion The expression of ATAD2 in HC increased,and the overexpression of ATAD2 was closely related to the occurrence,development,invasion,metastasis and poor prognosis of HC.
