摘要
目的 探讨初发系统性红斑狼疮 (SLE)病人Th1/Th2分布及其调控细胞因子、细胞因子受体基因表达的差异。方法 运用三色荧光标记法流式细胞术检测 42例未经药物治疗初发狼疮病人 ,T细胞亚群分布 ,并以 10名正常人作对照 ;同时运用ABI 770 0实时监测定量PCR法检测其中 38例病人和 2 8名正常人IL 10 /IL 12mRNA及其受体表达的差异。结果 ①初发SLE病人Th1较正常人明显减低 (P <0 0 5 ) ;但Th1/Th2无显著性改变。②与正常组相比 ,SLE组病人IL 10mR NA表达差异无显著性 ,但IL 10R表达明显升高 (P <0 0 5 ) ;SLE组病人IL 12mRNA及其受体表达较正常人明显降低 (P均 <0 0 5 )。③红斑组病人Th1/Th2、IL 12 p35较正常组降低 (P均 <0 0 5 ) ;IL 10R较正常组显著增高 (P <0 0 5 )。④RNP阳性组病人IL 10、IL 12p40较正常组升高 ,IL 12p35较正常组降低 (P均 <0 0 5 )。 结论 SLE是一种以Th1细胞下降 ,Th2细胞相对占优势的免疫介导的自身免疫性疾病 ,源于诱导向Th1细胞分化的IL
Objective To analyze Th1/Th2 balance of peripheral Th cells and its inducing cytokines and cytokine receptors in recent onset systemic lupus erythematosus (SLE) patients for investigating the cause of the immunological imbalance in SLE patients.Methods The intracellular cytokine detection method with flow cytometry was used to quantitate Th1/Th2 cells in recent onset SLE patients ( n =42) and the control ( n =10).At the same time,using ABI 7700 real time PCR to detect IL 10,IL 10R,IL 12 and IL 12Rβ 2 mRNA in SLE patients ( n =38/42) and the control ( n =28).Results There was no difference in the Th1/Th2 ratio between SLE patients and the control ( P >0 05).However,Th1 was decreased significantly in the SLE patient group than in the normal control ( P <0 05).Compared with the normal group,there was no difference in IL 10 between the SLE patient group and the normal control ( P >0 05) but IL 10R was significantly higher ( P <0 05);and in the patients with positive RNP IL 10 higher than in the normal control ( P <0 05).The cytokines including IL 12 p35,IL 12 p40 and their respective recptors IL 12Rβ 2 and IL 12 p35/IL 10 were all significantly decreased in SLE group than in the control ( P all<0 05),IL 12 p40 increased in the patients with positive RNP,however.SLE patients with rash had significantly lower Th1/Th2 ratio and IL 12 p35,but higher IL 10R mRNA expression.SLE patients with RNP positivity not only had higher mRNA expression of both IL 10 and IL 12 p40,but also had significantly lower IL 12 p35 expression.Conclusion SLE is a relative Th2 predominance disease due to the Th1 decrease and the IL 12 and IL 12Rβ 2 mRNA expression decrease too.SLE patients with rash and RNP positivity may be the special subtype of SLE,for they have different Th distribution and different imbalance of cytokine expressions
