心脏停跳与间断缺血心脏停搏心肌损伤的比较

摘要 目的 :通过检测不同方式冠状动脉旁路移植术 ( CABG)患者围术期血清心肌肌钙蛋白 I( c Tn I)的变化 ,以判断间断缺血心脏停搏的心肌损伤程度。 方法 :19例 CABG患者分为心脏停搏组及间断缺血组 ,于围术期 7个不同时间点取静脉血 ,测定 c Tn I含量。 结果 :两组患者转流前 c Tn I全部正常 ,术后全部升高 ,于心肌缺血结束后 8h达到峰值 ,并于心肌缺血结束后 168h恢复至正常 ;两组各时间点的血清 c Tn I无明显差异 ( P>0 .0 5 )。结论 :间断缺血心脏停搏条件下行 CABG围术期心肌损伤程度不重于心脏停跳下行 Objective: To evaluate perioperative myocardial injury by measuring cardiac troponin I(cTnI) in coronary artery bypass grafting(CABG) with different procedures. Methods: Nineteen CABGs were divided into cardioplegia group and intermittent ischemic arrest group. Venous blood samples were taken at 7 different time points during perioperative period for measuring cTnI. Results: cTnI levels were in normal range prior to operation and increased in all samples after operation, reaching peak values at 8 h, and still higher than the baseline value until 168 postoperative hour. cTnI levels were not significant different between 2 groups at same time point( P >0.05). Conclusion: Myocardial injury with intermittent ischemic arrest procedure is no more severe than that with cardioplegic arrest procedure during perioperative period.
