强直性脊柱炎患者疼痛调查分析

An investigation and analysis on pain in patients with ankylosing spondylitis

目的 :调查和分析强直性脊柱炎患者的疼痛情况。方法 :应用简化McGill疼痛调查量表对 93例强直性脊柱炎患者的疼痛情况进行调查 ,并进行测试后重复测试的重测信度检验、疼痛情况分析和疼痛程度与骶髂关节X线改变的相关性分析。结果 :简化McGill疼痛调查量表在评定强直性脊柱炎患者疼痛方面具有较好的重测信度 (相关系数r=0 .85— 0 .95 ,P <0 .0 5) ;强直性脊柱炎患者主要表现为胀痛、触痛和刺痛等慢性疼痛症状 ,疼痛强度为轻、中度 ;相当部分患者 (男性 35 .48% ,女性 54 .84% )有软弱无力的症状 ,这可能属于心因性疼痛 ,也可能是对疲劳症状的反映 ;强直性脊柱炎患者的疼痛与骶髂关节X线改变之间无显著相关性 (P >0 .0 5)。结论 :简化McGill疼痛调查量表评定强直性脊柱炎患者的疼痛信度较好。强直性脊柱炎患者主要表现为慢性、轻中度疼痛 ,且与影像学改变无显著关系。

Objective: To investigate and analyze on pain in adult patients with ankylosing spondylitis. Method: 93 adult patients with ankylosing spondylitis were participated. Each patient was assessed by the short-form McGill Pain Questionnaire. A correlation was observed between the pain and radiologic changes. Result: The test retest reliability of the short form McGill Pain Questionnaire was good ( interrater intraclass correlation coefficients were 0.85-0.95, P <0.05). The main types of sensory pain were heavy,tender and shooting. The main type of affective pain was tiring exhausting. The intensity scale were mild or moderate. There were no significant correlation between the pain and radiologic changes ( P >0.05). Conclusion: The short-form McGill Pain Questionnaire is reliable to qualitative assess the pain of patients with ankylosing spondylitis. The chronic pain appears to be significantly prevalent in patients with ankylosing spondylitis, and the pain intensity are mild or moderate. These results could help to clinically evaluate and physiotherapeutical treatment in patients with ankylosing spondylitis.
