鼻内窥镜下上颌窦双径路手术

ENDOSCOPIC MAXILLARY SINUS SURGERY WITH BOTH WAYS

目的 :探讨鼻内窥镜下双径路上颌窦手术的临床应用。方法 :鼻窦镜穿刺套管针分别从下鼻道、尖牙窝穿刺 ,在鼻内窥镜直视下切除上颌窦病变 ;对上颌窦病变较严重者循柯 -陆氏手术进路开放上颌窦自然窦口 ,以利引流。结果 :13例上颌窦囊肿 ,6例单纯性上颌窦霉菌性鼻窦炎患者均一次性治愈 ,9例鼻窦炎Ⅱ型患者 ,7例治愈 ,2例复发 ,再次手术治愈。结论 :鼻内窥镜下双径路上颌窦手术疗效好 ,并发症少 ,病人痛苦小且经济。

Objective To study the new clinical use of endoscopic maxillary sinus surgery with both ways. Methods To puncture from inferior nasal meatus and canine fossa with a nasal endoscopic needle, then, to cut away the pathological tissue of maxillary sinus under the endoscopic observation. By Caldwell-Luc approach, to open the natural ostium of maxillary sinus to the cases taking the more pathological maxillary, so as to having a good leading. Results 13 cases of maxillary cyst and 5 cases of non-allergic fungal sinusitis were cured only once. The seven of 9 cases taking sinusitis Ⅱ were cured only once, the others were operation again, and cured. Conclusions The new method has a good curative effect,less complications,less suffering and money.
