摘要
目的 进一步证实TCD用于检测偏头痛患者的脑部血流动力学 ,判断脑血管的功能状态 ,为临床诊断和治疗提供依据的准确性和可靠性。方法 应用TCD技术 ,对 5 1例健康人和 60例偏头痛患者进行多普勒检测 ,并对同一病例的发作期与间歇期脑血流动力学对比研究 ,提高了其可比性 ,减少了人为误差。结果 偏头痛患者的脑血流动力学有血流速增高 (87% )、血流速减低 (4 5 % )、血流速正常三种表现 ,另有血流速不稳和血管杂音等表现。并发现同一偏头痛患者血流速异常的血管及其异常部位发作期和间歇期不完全一致。结论 血流频谱异常及血管杂音可作为偏头痛诊断的佐证 ,典型偏头痛与普通型偏头痛两者均存在血管舒缩功能障碍 ,也说明偏头痛患者存在明显的植物神经功能紊乱。
Objective To apply TCD for detecting cerebral hemodynamics, estimating cerebrovascular function. Methods TCD examination in 51 cases of health and 60 cases of patients were carried out, and the same sufferer was contrastively analysed in outbreak period and diapause to advance the comparability and decrease the man made error. Results There were three changes of 60 migraineurs' cerebral hemodynamics:①Velocity of blood flow accelerated (87%).②Blood flow velocity decelerated (45%).③Blood flow velocity was normal, aside the blood flow velocity's unstable and asymmetrical or vascular bruit, et. In the same patient, in effect time and diapause, the abnormal vascular and position was incompletely accordant. Conclusion These factors were ascertainable diagnosis, and which explained both TM and CM groups occurred vascular function obstruction. The migraineur subsisted evidence of autonomic nerve dysfunction.
