髁上逆向交锁髓内钉治疗陈旧性股骨髁上骨折合并膝关节僵硬

Retrograde intramedullary supracondylar nail in the treatment of old supracondylar fracture of femur combined with stiff knee joint

目的 介绍用髁上逆向交锁髓内钉治疗陈旧性股骨髁上骨折合并膝关节僵硬的方法与疗效。方法 对 10例患者采用手术松解粘连、矫正畸形、股骨髁上逆向交锁钉内固定治疗 ,术后早期应用CPM机锻炼。结果 10例均随访 6～ 2 0个月 ,肢体无畸形 ,骨折愈合时间 6～ 15个月 ,平均 8个月 ,优良率达 90 %。结论 应用股骨髁上交锁钉治疗陈旧性股骨髁上骨折合并膝关节僵硬操作简便、固定牢靠、患者可早期功能锻炼 ,效果良好 。

Objective To introduce clinical applications and effects of retrograde intramedullary supracondylar(RIMSC) nail in the treatment of old supracondylar fracture of femur combined with stiff knee joint. Methods 10 cases were treated with RIMSC nail and arthrolysis were done. All cases accepted CPM exercise as early as possible after operation. Results All cases were followed up for 6 to 20 months. No limb deformity was found and all fracture united within 6 to 15 months (8 months on average) after operation. The good and excellent rate was up to 90%. Conclusion The use of RIMSC nail in treatment of old supracondylar fracture of femur combined with stiff knee joint has the advantage of simple performance, solid fixation and early exercise so that it becomes a more effective therapy.
