高频彩超诊断乳腺癌腋窝转移性淋巴结的临床价值

The clinic value of color doppler ultrasound in diagnosing of axillary metastatic lymph nodes

目的 评价彩超与临床触诊在诊断乳腺癌腋窝转移性淋巴结中的价值。方法 60例乳腺癌行腋窝淋巴结清扫术患者作术前前瞻性彩超检查 ,并和术后病理对照 ,双盲法观察分析影像所见 ,将其结果做统计学处理。结果 在 60例乳腺癌腋窝淋巴结清扫术后病理检查中 3 3例有转移性淋巴结。临床触诊检出其中 1 7例 (敏感性 5 1 .5 %、特异性 88.9%、准确性 68.3 % ) ,彩超检出 2 8例 (敏感性 84.8%、特异性1 0 0 %、准确性 88.3 % )。彩超可确诊 68.7%临床漏诊的隐匿性转移淋巴结。结论彩超明显优于临床检查 。

Objective To evaluate the accuracy of color doppler ultrasound and manual palpation in the diagnosis of axillary metastatic lymph nodes (AMLNs) and to identify the value of ultrasound in the diagnosis of occult (AMLNs).Methods Sixty patients were studied double \|blindedly by color Doppler ultrasonography prior to axillary lymph\|adenectomy, and the results were correlated with histopathologic findings.Results According to histologic examination of the dissected specimens, in 33of 60 patients were axillary lymph node metastases , 17of 33 patients with metastatic nodes were diagnosed by manual palpation (sensitivity ,51.5%; specificity, 88.9%; accuracy , 68.3%). However, lymph node metastases in 28 patients were revealed by color Doppler ultrasound (sensitivity ,84.8%; specificity ,100%; accuracy, 91.7%), color Doppler ultrasound discovered 68.7% of the occult metastatic nodes in this series .Conclusion Color Doppler ultrasound examinations are superior to physical palpation in ascertaining (AMLNs) ,as 68.7% of the occult (AMLNs) can be diagnosed.
