Effects of BCL-6 expression on formation of lymphoid germinal center and pathogenesis of lymphoma

目的 探讨 BCL - 6在正常淋巴组织和一组淋巴瘤中的表达与生发中心形成及不同类型淋巴瘤发生的关系 .方法 形态学及免疫组织化学方法对 17例反应性增生淋巴组织(RL H)及 74例不同类型淋巴瘤进行检测并分类 ,免疫组化法检测各组织石蜡标本中 BCL - 6表达情况 .结果 17例 RL H生发中心细胞、10例滤泡性淋巴瘤 (FL)及 4例弥漫性大 B细胞淋巴瘤 (DL BCL )中等强度表达 BCL - 6 ;3例 FL及 2 1例 DL -BCL BCL- 6表达为强阳性 (72 % ) ,高于其他各组 (P<0 .0 1) .17例 RL H生发中心外细胞 ,4例 DL BCL ,14例小淋巴细胞淋巴瘤 (SL L)、4例套细胞淋巴瘤 (MCL)及 14例经典型霍奇金淋巴瘤 (HD) BCL- 6阴性 .结论 BCL- 6表达与生发中心形成密切相关 .少数 FL和大部分 DL BCL 中 BCL- 6过表达 ,可能与这些淋巴瘤发病机制有关 .检测 BCL- 6有助于 FL 与 SL L,MCL间及 RL H与

AIM To study the effects of bcl 6 expression on the formation of lymphoid germinal center and the pathogenesis of different types of lymphomas. METHODS Histomorphology and immunophenotype of 17 cases of lymph node reactive hyperplsia (RLH) and 74 cases of lymphoma were detected and classified. BCL 6 was detected in both paraffin sections of RLH and lymphoma with immunohistochemistry. RESULTS BCL 6 was expressed moderately in cells within the germinal center of the 17 cases of RLH, in 10 cases of follicular lymphoma (FL) and in 4 cases of diffuse large B cell lymphoma (DLBCL). Three cases of FL and 21 cases of DLBCL were found to express BCL 6 intensively, with statistic significance in positive intensity between DLBCL and above mentioned groups ( P <0.01), while BCL 6 was negative in cells outside the germinal center of the 17 cases of RLH, in 14 cases of small lymphocytic lymphoma (SLL), in 4 cases of mantle cell lymphoma (MCL) and in 14 cases of classic Hodgkin's lymphoma (HD). CONCLUSION There is a close relationship between BCL 6 expression and the formation of germinal center. The overexpression of BCL 6 in some cases of FLs and DLBCLs may play a key role in their pathogenesis. It may be helpful to detect BCL 6 in distinguishing FL from SLL and MCL and in distinguishing RLH from HD.
