纤维蛋白胶干粉对大鼠出血模型的止血作用

Hemostatic efficacy of a fibrin sealant powder in rat models

目的 :评价新型药物纤维蛋白胶干粉对大鼠肝损伤和股动脉切口模型的止血效果。方法 :采用SD大鼠 ,雌雄各半 ,麻醉后注射肝素抗凝 (30 0U/kg) ,然后制造肝损伤或股动脉切口模型 ,分别用 4种方法进行处理 :①不处理 ;②纱布压迫处理 ;③凝血酶 (临床用生物止血药 )加纱布压迫处理 ;④纤维蛋白胶干粉加纱布压迫处理。结果 :对肝损伤模型 ,组④和组③能在 1min和 3min内止血 ,组①和组② 30min内不能止血 ,组④和组③的失血量和血压降低百分率与组②比较均有非常显著的差异 ,组①与组②比较只有血压降低百分率有显著性差异 ;对股动脉切口模型 ,组④可以在 3min内止住血流 ,组①、组②和组③均不能在 30min内止血 ,组④的失血量和血压降低百分率与组②间有非常显著的差异。结论 :纤维蛋白胶干粉的止血效果优于凝血酶和常规纱布处理 ,对急性动脉出血优势更明显 ,有望开发成急性创伤和临床手术中的高效止血药。

Objective:To determine the hemostatic efficacy of a fibrin sealant powder as compared with a hemostatic-thrombin and a gauze compression control in animal models of liver and femoral artery injury.Methods:Adult SD rats were anesthetized and underwent partial liver resection or femoral artery incision after heparin anticoagulation (300?U/kg intravenous). The animals were randomized to four groups according to the methods of treatment:group Ⅰ, no treatment; group Ⅱ, gauze compression; group Ⅲ,thrombin plus gauze compression; group Ⅳ, fibrin sealant powder plus gauze compression.Results:In liver injury model, blood loss (ml) was less in group Ⅳ(0.39±0.26) than in group Ⅲ(1.08±0.58), group Ⅱ(3.38±0.89) and group Ⅰ (3.69±0.81) ; the percentage decrease (%) in the mean arterial pressure was less in group Ⅳ(5.89±0.38) than in group Ⅲ (6.27±9.81) ,group Ⅱ(38.88±23.42) and group Ⅰ(62.1±17.41).In femoral artery injury model, blood loss (ml) was less in group Ⅳ(2.10±0.91) than in group Ⅲ(5.60±0.83), group Ⅱ(5.96±1.25) and group Ⅰ(6.38±1.10); the percentage decrease (%) in the mean arterial pressure was less in group Ⅳ(43.30±28.35) than in group Ⅲ(100.00±0.00), group Ⅱ(100.00±0.00) and group Ⅰ(100.00±0.00).Conclusions:Fibrin sealant powder provides effective hemostasis and is superior to gauze compression and thrombin in rat models. Further development of fibrin sealant powder for potential clinical use is warranted
