应用脉冲多普勒组织成像测量冠心病左室局部等容舒张时间的价值 被引量:3

A study on regional isovolumic relaxation time of left ventricle in patients with CAD by pulsed-wave Doppler tissue imaging

摘要 目的 探讨脉冲多普勒组织成像 (PW -DTI)测量冠心病患者局部等容舒张时间 (R -IVRT)的可行性及R -IVRT的临床应用价值。方法 应用PW -DTI技术对 32例正常人 (正常组 )和经冠状动脉造影证实的 34例冠心病患者 (冠心病组 )左室 16节段的R -IVRT进行测量 ,比较冠心病组和正常组相应节段的测量值 ,同时每个研究对象 16节段R -IVRT的均值 (GRI)与左室整体等容舒张时间 (G -IVRT)、二尖瓣血流的E/A比值进行比较。结果 正常组各节段R -IVRT无差异 ,而冠心病组的病变节段长于正常组的相应节段 (P <0 .0 5 )。GRI与G -IVRT存在相关 ,正常组和冠心病组的相关系数分别为 0 .44 8、0 .45 0 (P <0 .0 1)。GRI与二尖瓣血流的E/A不相关 ,正常组和冠心病组的相关系数分别为 - 0 .181、- 0 .0 85 (P >0 .0 5 )。结论 PW -DTI可快速、无创地测量冠心病者的R -IVRT ,该指标可作为评价局部功能的较好指标 ,但是否为负荷依赖性尚须进一步研究。 Objective To study the feasibility of assessing regional isovolumic relaxation time (R-IVRT) in patients with coronary artery disease(CAD)by pulsed-wave Doppler tissue imaging (PW-DTI) and the clinical value of R-IVRT. Methods The R-IVRT of 16 segments in 32 control subjects and 34 CAD subjects, who all were identified by coronary artery angiography, was measured by PW-DTI. Comparisons were carried out between the mean value of R-IVRT of 16 segments of each subject (the global isovolumic relaxation index, GRI) and global isovolumic relaxation time (G-IVRT), and between GRI and E/A of mitral blood flow. Results The R-IVRT in control group showed no significant difference between different segments, but the R-IVRT of diseased segments in CAD group was longer than that of the corresponding ones in control group (P<0.05). There was a weak correlativity between GRI and G-IVRT in control and CAD groups (r=0.448 and 0.450 respectively, P<0.01), but there was no correlativity between GRI and E/A of mitral blood flow (r=-0.181 and -0.085 respectively, P>0.05). Conclusion R-IVRT in CAD subjects can be measured by PW-DTI quickly and non-invasively. R-IVRT is a better parameter to assess regional diastolic function, but whether it is load-independent or not is worth further studying.
