睡眠脑电图对儿童癫痫的诊断价值

Diagnostic value of sleep EEG of epilepsy in children.

目的 :探索睡眠过程中脑电图变化与癫痫的诊断价值。方法 :通过88例小儿癫痫常规、睡眠脑电图检查 ,同时观察癫痫临床与清醒睡眠脑电图变化的联系。结果 :睡眠脑电图比常规脑电图痫样放电率明显提高 ,尤其在睡眠Ⅰ、Ⅱ期 ,同时记录到癫痫发作起始时波形变化 ,脑电起源部位、扩展与传导 ,并与临床发作同步观察 ,尤对复杂部分性发作、全身强直发作有效 ,以儿童颞叶癫痫为著。结论 :睡眠脑电图可明显提高癫痫的阳性率 ,并对小儿癫痫诊断、分型、确定部位、发作频率监测、指导用药。

Objective:To explore the change of sleep EEG and diagnostic value of epilepsy.Methods:The routine EEG and sleep EEG were determined in 88 patients with epilepsy,simultaneously the relationship between clinic epilepsy and awakening EEG and sleep EEG was observed.Results:The epileptiform discharge rate of sleep EEG increased significantly comparing to the routine EEG,especially in the sleep period Ⅰ and Ⅱ.The wave change at begining of epilepsy seizure,the origin region,expanse,conduction of encephalon electriaty were recorded,the clinical seizure of epilepsy also was observed.The epileptiform discharge rate was even more effective in chicdren with partial complex and general tetanic seizure,especially in tempora epilepsy.Conclusion:Sleep EEG can raise the positive rate of epilepsy,which is a certain value for diagnosing,classifying,determining region,monitoring the frequency of seizure,guiding the clinical administration,and juding the prognosis of epilepsy in children.
