口腔颌面部修复组织瓣的放射反应观察

A clinical report of the radiatied flap in oral-maxillofacial region

目的 :探讨术后放疗过程中口腔颌面部各类修复组织瓣的放射反应表现。方法 :观察 2 9例带有 31块口腔颌面部修复组织瓣的术后放疗患者 ,参照RTOG国际标准对各类组织瓣的急性放射反应进行分级。结果 :口内外各类组织瓣均出现类似于正常组织的放射反应 ,同时也有一些特殊表现 ,但无一例坏死。结论 :口腔颌面部各类修复组织瓣对术后常规放疗都具有良好的放射耐受性。

?Objective:To observe the reaction of flaps following postoperative radiation therapy because of malignant tumor.Methods:31 flaps in 29 patients were used to repaire the defects after removal of malignant tumor in oral maxilloacial region.Radiation therapy of 60 Co at the dosage of 4 880 7 100 Gy was given to the patients 2 6 weeks after operation.Fowllow up was conducted weekly till 9 months following the radiation therapy and the reaction of the flaps was clinically observed and judged according to the criteria of RTOG (radiation therapy oncology group).Results:Ⅰ,Ⅱ,Ⅲ and Ⅳ reactions were observed in 6,2,4 and 0 of the 25 intro oral flaps,and in 1,14,5 and 1 of the control oral mucosa in the idential individuals respectively ( P <0.05).Those were observed in 3,2,0 and 0 of the 9 out oral flaps,and 3,3,1 and 0 of control skin in the identical individuals respectively ( P >0.05).Reactions induced by the radiation at the dosages of less than 5 000 Gy ,5 000 5 900 Gy and more than 6 000 Gy were not significantly defferent ( P >0.05).Conclusion:The flaps in oral maxillo facial region are tolerant toward regular radiotion therapy.
