摘要
目的探索正常人肾皮质及慢性肾病肾皮质的超声弹性系数是否有相关性;探索肾皮质超声弹性系数在肾纤维化不同阶段的临界值。方法收集肾脏穿刺活检的患者76例和健康志愿者40例,分别进行声触诊组织定量分析(VTQ),测量其横向剪切波速度(Vs)。结果测定的Vs值与肾间质纤维化程度之间具有较好的相关性,随着肾间质纤维化程度的加重,肾皮质Vs值降低。剪切波速度(Vs)数值以2.45m/s为临界值时,ROC曲线下面积最大,对诊断肾间质纤维化意义最大(准确度93%)。结论声脉冲辐射力成像技术(ARFI)是一种无创、可靠的评估肾间质纤维化的方法,有助于评估慢性肾病的病变程度。
Objective To investigate ultrasound elasticity coefficient of normal renal cortex and chronic kidney disease wheth- er there is a correlation,and we investigated the critical value of ultrasonic elasticity coefficient in the renal cortex renal fibrosis at different stages. Methods We selected 76 patients who underwent renal biopsy and 40 healthy volunteers, these subjects were underwent VTQ, and then we mesured transverse shear wave velocity (Vs). Results There was a correlation between Vs and the stage of Renal interstitial fibrosis, the value of Vs would decrease along with the deterioration of the renal interstitial fi- brosis. When the critical value of the Vs was 2.45m/s, the area under the receiver operating curve(ROC) was the biggest. Its diagnosis of renal interstitial fibrosis was better. {Its accuracy was 93 % ). Conclusion Acoustic radiation force impulse imaging (ARFI) can reflect the renal interstitial fibrosis and it is reliable and noninvasive, so this technique can evalute the degree of chronic kidney disease.
