中枢神经系统血管外膜细胞瘤的组织学及免疫组织化学研究 被引量:10

Histological and immunohistochemical studies on haemengiopericytomas in the central nervous system

摘要 目的 研究脑脊膜血管外膜细胞瘤的诊断、组织来源、增殖与复发的关系。方法 对5 3例肿瘤进行HE染色及 6种抗体的免疫组织化学染色 ,光镜下观察。结果 初发 2 7例 ,复发 2 6例 ;组织学表现为以血管为中心的血管外膜细胞增生 ,血管腔圆或裂隙样 ,肿瘤细胞圆或不规则 ,Ⅱ级 35例 ,Ⅲ级 18例 ;FⅧ 5 3例血管内皮细胞阳性及散在少许肿瘤细胞阳性 ,Vimentin 5 3例肿瘤细胞胞浆及血管内皮细胞阳性 ,PCNA LI 2 5 82 4 % ,平均值 4 8 85± 19 2 6 ,P5 3 LI 0 2 9% ,平均值 0 5 5± 0 4 8。结论 脑脊膜血管外膜细胞瘤是独立的恶性肿瘤 ;复发与其组织学分级、PCNA和P5 3表达无明显关系 ;该肿瘤可能来源于未分化间叶细胞 ,这种细胞有向血管内皮和血管外皮细胞分化的潜能。 Objective To study diagnosis?histogenesis and relationship between proliferation and clinical recurrence of meningeal haemaniopericytomas(HPC). Methods 53 cases of HPC were studied by HE and immuno histochemical staining with six kinds of antibody, observed under LM. Results Incipient 27 cases, recurrence 26 cases. HPC were characterized by haemangiopericytes range around the vessles. The cells are oval or elonged, vascular spaces dilated staghorn type, degreeⅡ35cases, degreeⅢ18cases; FⅧ and Vimentin were positive in all cases. PCNA LI was 48 85±19 26 with a range of 2 5 82 4%. P53 LI was 0 55 ±0 48 with a range of 0 2 9%.Conclusions Meningeal HPC was maglignant neoplasm. There was no clear relationship between recurrence and grade?PCNA?P53. HPC may arise from the undifferentiated mesenchymal cells that have potentiality of differentiating into haemangiopericyte and haemangioendotheliocyte.
