关节镜下冲洗与透明质酸钠注射治疗骨性关节炎的临床研究

Clinical Study on the Management of Osteoarthritis with Lavation and Natrium Hyaluronic Acid Injection under Arthroscope

目的 探讨关节镜下冲洗后关节腔注射透明质酸钠对骨性关节炎的临床疗效及对血清与关节液中IL - 1、NO水平的影响。方法 38例有膝关节腔积液的骨性关节炎患者随机分成两组 ,给予关节镜下冲洗后分别行关节腔注射透明质酸钠治疗 (治疗组 )和非甾体药治疗 (对照组 ) ,观察治疗前后关节疼痛 (静息痛、运动痛 )、关节腔积液及肿胀与关节功能改善程度。同时测定治疗前后血清与关节液中IL - 1、NO水平变化。结果 两组病例治疗后各项观察指标均有明显改善。但在治疗停止后 1年 ,治疗组的疗效优于对照组 ,其差异有显著性。结论 关节镜下冲洗并透明质酸钠注射联合治疗早、中期骨性关节炎有明显疗效。

Objective: To explore clinical effects of lavation and intra-articular injection with natrium hyaluronic acid on osteoarthritis and its influences on the level of IL-1,nitric oxide in serum and synovial fluid.Methods: 38 osteoarthritis patients with knee joint hydrarthrosis were randomly divided into treatment group and control group,which received intra-articular injection with natrium hyaluronic acid and non-steroid anti-inflammatory drug therapy respectively after lavation of knee joint.And then the various indexes of articular resting pain,active pain,hydrarthrosis, swelling, function and level of IL-1,nitric oxide in serum and synovial fluid were recorded respectively before and after therapy.Results: The indexes of the two groups were greatly improved. After one year when the treatment stopped, the effect of the treatment group was superior to the control group. There was significant difference between the treatment group and control group. Conclusion: lavation of knee joint combined with intra-articular injection with natrium hyaluronic acid has significant effect on early and medium-term osteoarthritis.
