腭咽扩张成形术治疗阻塞性睡眠呼吸暂停综合征

The investigation of effect of uvula palato-pharyngo-dilation-plasty surgery on 168 patients with obstructive sleep apnea syndrome

目的:探讨阻塞性睡眠呼吸暂停综合征(ＯＳＡＳ)手术方法的改进,提高疗效,减少术后并发症。方法:对传统术式进行改进,设计腭咽扩张成形术治疗ＯＳＡＳ患者１６８例,术后进行半年以上随访。结果:本术式较好的解决了口咽狭窄所致的响鼾及睡眠呼吸暂停,有效提高了患者睡眠质量,１６８例中治愈１２３例(７３．２%),显效４３例(２５．６%),无效２例１．２%)。结论:本术式可有效增加软腭紧张度,扩大口咽腔通气面积,尽可能保留咽粘膜正常功能,减少术后并发症。

ve: To investigate the effect of uvula-palato-pharyngo-dilation-plasty (UPPDP)surgery on patients with obstructive sleep apnea syndrome(OSAS). Methods :One hundred sixty eight patients were diagnosed and treated by surgery of UPPDP method from June 1992 to December 1999. The approach of UPPDP surgery was derived from traditional operative method. All patients were followed up in half year after treatment. Results:Among 168 patients, 123(73. 2%)were cured, 43(25- 6%) effective and 2(1. 2%) not effect. The complications of this operation were not found. Conclusion: UPPDP surgery is one of effective treatment methods for patients with OSAS which can relive sleep apnea resulted from pharyngostenosis and improve the quality of patients sleep . Our result showed that the advantage of UPPDP should be effectively increase the palate tension and enlarge the area of pharyngo-oral ventilation, to preserve the normal function of pharyngeal mucous membrane.
