彩色多普勒能量图探测胆囊肿瘤血流的临床意义

Vascularity index as a novel parameter for in vivo assessment of angiogenesis in patients with gallbladder tumors

为探讨彩色多普勒能量图 ( CDE)检测胆囊肿瘤血流的临床意义 ,对 3 6例胆囊癌和 2 6例胆囊腺瘤患者于术前行 CDE检测 ,对术后切除标本行微血管密度 ( MVD)和流式细胞计数检查。结果显示 ,CDE显示的胆囊肿瘤信号以 型、 型为主 ,胆囊癌以 型、 型为主 ( P<0 .0 5 ) ;CDE显示的胆囊腺瘤血流信号与 MVD测定肿瘤血管数目和分布呈正相关 ( P<0 .0 5 ) ,且与肿瘤大小、侵润转移及肿瘤内血流频谱的峰值速度 ( ps)有关 ( P<0 .0 5 ) ;CDE显示血流信号为 型、 型者 ,发生淋巴结转移及 S+ G2 M期指数目明显高于 型、 型 ( P<0 .0 5 )。认为 CDE能够在体外评价胆囊肿瘤血管生成 ,是鉴别胆囊良恶性肿瘤和判断肿瘤预后有用的参数。

To assessd angiogenesis by color Doppler power imaging in vivo,hope to develop an noninvasive method for quantiating gallbladder tumor angiogenesis Color Doppler power imaging(CDE)was performed before sugery to serch for blood flow signals from 36 case of gallbladder carcinomas and 26 case gallbladder adenomas The microivessel density(MVD)and Flow cytometry(FCM)of the excised tumor was assessed Clinical and pathologyic data also were recorded,pattern of blood flow in gallbladder abdenomas mailly exhibiting CDE Ⅰ,Ⅱ;and in gallbladder carcinomas was CDEⅢ,Ⅳ(P<0 05) The numers of angiogenesis in CDE was corelationship in MCD,tumor size,metastasis and PS(P<0 05) Patients with CDE Ⅲ?Ⅳ exhibited significantly more lymph node metastasis and s +GM phase fraction than those with CDE Ⅰ?Ⅱ CDE can fully show the blood treee of the tumors,assessed angiogenesis in vivo It could be a useful parameter for distinguishing the maglinant gallbladder tumors from the benign ones and promising promising predictor fot the prognosis of patients with gallbladder tumors
